Our Adult Day Health Center (ADHC) is seeking a dedidcated Social Worker to join our team to provide assessment and case management services to elderly and other physically/mentally impaired adults in day program setting. Responsible for assisting the team to provide all social work services at center. Will conduct assessments, reports, individual and family counseling as needed, in addition to standard case management duties (telephone calls to caregivers, progress notes, developing/following treatment plans).
Qualifications:
-Master's or Bachelor's degree in social work, psychology, or sociology
-Microsoft Office computer programs (additional computer program training will be provided)
-Must be energetic, self-motivated and efficient with completing tasks and meeting deadlines
-Knowledge of community resources in Los Angeles and surrounding areas a plus
-Mandarin or Cantonese speaking
Job Types: Full-time, Part-time
Pay: $32.00 - $36.00 per hour
