We are seeking a Home Health Occupational Therapist to join our team You will provide direct patient care and support rehabilitation activities.

**** Must be licensed with a minimum 2 years clinical experience in any environment ****

**** We are hiring for FULL TIME & PART TIME positions ****

Responsibilities:

  • Facilitate rehabilitation of patients to maximize patient's independence
  • Develop & implement treatment plans for rehabilitation
  • Communicate patients response to treatment to all relevant parties
  • Document all care in patient records
  • Active participation in case conferencing & interdisciplinary communication

Qualifications:

  • Previous experience in occupational therapy
  • Ability to build rapport with patients
  • Passion to help and motivate others
  • Ability to thrive in a fast-paced environment
  • Excellent written and verbal communication skills
