Innovative services offered across Chicagoland and Central Illinois include community living options, novel day programs, behavioral health services, employment programs, respite for families, and foster care for children with disabilities.


Purpose:

The Host Family Respite will evaluate and provide training to residents in daily living and social skills in order to assist them in achieving and maintaining their maximum level of functioning with independent community living.


Essential Responsibilities, include but are not limited to:


Monitor and assist residents with personal grooming and appearance as necessary.

Transport and accompany residents to appointments and other services, including day program sites, as needed.

Supervise and assist residents in taking their medication in accordance with program guidelines.

Assist Caregivers with vacation days, but caring for the individual/ or individuals inside of homes.

Assist individuals to and forth social activities, and other meetings and appointments.

Respond to medical and behavioral emergencies following appropriate Health and Safety, Aggression Management, First Aid, CPR, and other procedures as necessary.

Plan menus and assist with meal preparation as required. Monitor and observe residents with special dietary needs.

EDUCATION, SKILLS, and/or EXPERIENCE:


High school diploma and/or technical school/GED equivalent

Valid Drivers License

Pass a full criminal background check.

Must be able to work independently.
