Position Objective: The Program Coordinator / QIDP ensures individuals with developmental and intellectual disabilities receive supports and services to live their best lives. They are responsible for providing counseling, case management, and structured behavior programming to people with disabilities receiving residential services. Additionally, the Program Coordinator is responsible for the implementation of Personal Plans as well as following rules and regulations as required by licensing entities.

Essential Duties/Responsibilities

Mission, Vision, Values

  1. Support the philosophy through the agency’s mission, vision, values, and strategic plan.
  2. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.
  3. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

Program /Department Development, and Coordination

  1. Provide Quality Assurance for the condition of homes through inspections as needed.
  1. Work with all members of a person’s team to include but not limited to other EPC team members, guardians, and ISC.
  2. Assure all safety and emergency procedures are conducted in a calm, accurate and professional manner and documented, as necessary.
  3. Participate in discovery meetings for all people on their caseload.
  4. Act as a positive role model for others.
  5. Assure dignity, privacy and rights of people are always observed.
  6. Assure that activities are developmentally and age appropriate and follow principles of normalization.
  7. Assure that people’s needs and interests are assessed on an ongoing basis.
  8. Assure that the above findings are utilize for planning for each person.
  9. Assure that personal and/or social development of people supported occurs.
  10. Maintain a QIDP caseload as needed when changes occur with QIDP caseloads as designated by Director.
  11. Coordinate the admission, transfer, and discharge as needed for people.
  12. As a mandated reporter, ensure allegations of abuse, neglect and exploitation are reported as required by EPC policy and government regulation.
  13. Conduct prompt and thorough investigations for all allegations documenting as required.
  14. Participate in meetings, committees, conferences, seminars, and in-service training sessions as requested.
  15. Participate in community events, presentations and educational opportunities.

Fiscal Management

  1. Use resources responsible.
  2. Work with supervisor to seek approval of expenses as required.

Database, Paperwork and Compliance Responsibilities:

  1. Meets all deadlines set by the agency and outside resources.
  2. Complete monthly summaries and ensure all documentation is completed to meet clinical and financial requirements.
  3. Maintain files according to standards set by the agency, ICAPs, consents, assessments, and other needed paper or electronic documents.
  4. Review all documentation for accuracy.
  5. Understand rules governing program/licensure for CILA, monitors the implementation to and adherence of these rules.
  6. Complete and required accounting procedures in a timely manner.
  7. Supports database management systems.
  8. Assist with OIG investigations.

Resource Management Responsibilities:

  1. Provide support to and work with all team and agency members.
  2. Maintain positive guardian, family and natural support relations for people supported.
  3. Promote community participation for people.
  4. Review all medical and other incident reports for the person.
  5. Coordinate and support appointments with the nursing department.
  6. Work with ISC’s.
  7. Assist in screenings for people interested in accepting support.
  1. Assist new providers and individuals in the transition process.
  1. Communicate and serve as an agency liaison to parents, people we support, peers, community, neighbors, and work groups.
  2. Develop and maintain natural support systems and promote community participation for individuals.
  3. Audit Provider compliance with contractual responsibilities and meeting required operational standards during home visits.

Clinical and Behavioral Management Responsibilities:

  1. Effectively facilitate and lead EPC team meetings so outcomes are clearly identified and recorded.
  2. Implement Person-Centered Plans (PCP) for all people on caseload, including writing and entering implementation strategies, assist in facilitating yearly Discovery meetings, along with other meetings as appropriate.
  1. Communicate regularly with people supported, family/guardian, nursing, and other team members regarding progress toward outcomes, health and safety, and other issues that may arise.
  1. Achieve desired outcomes based on individual choice as delineated in the person’s PP
  2. Assist with compatibility assessments with people on an ongoing basis.
  3. Provide training (OJT’s) and coaching to the contractual providers and agency staff in the principles and practices of Implementation Strategies.
  4. Train staff and providers on Behavior Plans/Interventions related to the person.
  5. Develop behavior plan for people who are need of behavioral support from their support team.
  6. Work with all members of a person’s team to include but not limited to other EPC team members, guardians, ISC, BCBA, and psychiatrist.
  7. Audit functions of each contractual provider’s home monthly to ensure individual’s needs are being met.
  8. Participate in the Behavior Management Committee and Human Rights Committee as needed.
  9. Participate in all needed appointments with a person’s medical team and psychiatrist.
  10. Participate in the on-call rotation.
  11. Assist with investigations as requested.
  12. Participate in Community Options vacations, power team activities and other events that support the Community Options program.
  13. Participate in the on-call rotation.

Clinical Management Training:

  1. In accordance with agency policy and procedures, support the agency’s DSP training in role modeling, coaching, mentoring, and participating in on-the-job training with program staff.
  1. Train all aspects of clinical responsibilities, including incident reports, Personal Plan, monthly summaries, drill sign off, and other clinical tasks.
  1. Provide training and retraining as needed for providers and agency personnel in the efficient use of electronic data entry and communication (Therap, ECP & Quantum)
  2. Provide training and coaching to the contractual providers and agency staff in the principles and practices of person-centered planning.

Personal and Professional Development:

  1. Participate in meetings, committees, conferences, seminars, and in-service training sessions as requested.

Personnel Relations:

  1. Interact with all departments and EPC staff, with individuals from monitoring agencies/committees, parents, family members, and other professionals in a professional manner.
  2. Facilitate intra-and interdepartmental communication on an ongoing basis.
  1. Provide back up to other CO team in QIDP role.

Supervisory Responsibilities:

  1. Requires On-Call responsibilities.

Marginal Responsibilities

  1. Perform other duties as assigned.
  2. As a mandated reporter, ensure allegations of abuse, neglect, and exploitation are reported as required by EPC policy and government regulation

Education/Experience Requirements

  1. Bachelor’s Degree in Human Services required, and two years supervisory experience preferred.
  2. Supervisory experience is preferred.
  3. One year experience with persons with developmental disabilities.
  4. The ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write correspondence.
  5. Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
  1. Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker’s Registry under the Healthcare Worker’s Background Check Act.
  2. Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.
  1. Achieve and maintain QIDP status per DHS regulations.
  2. Ability to effectively present information in one-on one and small group situations to customers, clients, and other employees of the organization.
  3. Must possess emotional stability to handle stressful situations on a regular basis and the ability to cope with illness, death, and dying.
  4. Must possess good organizational and time management skills.

Work Environment

  1. Work may occur in the following settings: Home, Office, Classroom, Host Home Site, Group Homes, or Community.
  2. Participate in a 24 hour on-call system.
  3. Work hours/days are full-time and exempt status. Flexibility is a requirement to accommodate various Program, contractual, personnel and organizational needs.
  4. Will be required to drive a motor vehicle (EPC’s and employee’s) as part of his/her employment responsibilities with EPC. A valid Illinois Driver’s License and insurance is required and must be maintained at all times while employed by EPC.
  5. Be a team player with a positive attitude towards the people EPC supports, peers, EPC staff and outside contacts.
  6. May be required to work extended shifts and/or be called upon for emergencies.
