Prevention Case Management Specialist

The Prevention Case Management Therapist will work as a member of the multi-disciplinary healthcare team to provide primary and secondary prevention case management services to all eligible patients.
The Prevention Case Management Therapist provides patient screening/assessment, risk reduction education and counseling (i.e. sexual behaviors, substance abuse, mental health) and links the individual to appropriate providers, agencies and programs for mental health and substance abuse treatment assistance. Provides therapy in individual and group settings.
Key Responsibilities:
- Identify available resources and inform patients about available resources
- Provide education on accessing primary medical care and specialty care services and prescription assistance services
- Coordinate referral services and assist with transportation to services
- Coordinate the overall process to ensure services are timely, affordable and of high quality
Primary Prevention:
Reduces behaviors that place the patient at risk for HIV-STD infection. Primary prevention focuses on the reduction of the behaviors that place the patient at risk for HIV re-infection and/or transmission of HIV to another individual.
Primary Prevention Goals:
- Negative persons of greatest risk
- Newly diagnosed individuals
- HIV+ patients that engage in high-risk behaviors
Causes of High-Risk Behaviors:
- Lack of knowledge
- Cultural beliefs
- Substance abuse
- Sexual activity and substance abuse combined
- Mental health disorders
- Relationship/family issues
- Socio-economical conditions
Secondary Prevention:
Ensures adherence for the prevention of treatment of opportunistic infections and adherence to complex antiretroviral combination therapies. Secondary prevention is focused on maintaining the health of the client.
Secondary Prevention Goals:
- HIV+ patients that are at risk for non-compliance to medically necessary primary medical and/or specialty care and/or counseling and/or mental health related treatments
Causes of Non-Adherent Behaviors:
- Lack of treatment knowledge
- Untreated side effects of therapy
- Cultural health beliefs
- Substance abuse
- Mental health disorders
- Relationship/family issues
- Socio-economical conditions
Environmental Hazards:
Possible exposure to hazardous materials, hot and cold, blood-borne pathogens. Infrequent exposure to contaminated blood and other bodily fluids, communicable diseases.
Physical Requirements:
Ability to lift, carry, push, and pull up to 25 pounds, stoop, reach, stand, walk, finger, grasp, feel, talk, hear, see, and perform repetitive motions with or without reasonable accommodations.
