Bay Nursing Inc., a well established Home Care Agency, and market leader in home nursing and therapy services, located in Romeo, is looking for a full time Licensed Master Social Worker to join our Home Health and Hospice Team.

Bay Nursing Inc., has provided our patients a comprehensive range of home health care medical services including skilled nursing, physical and occupational therapy for over 30 years and now provide Hospice Care. We are committed to excellence in patient care and a supportive work environment.

Experience in home health care preferred. Must hold a current valid Licensed Master Social Worker License in MI.

Duties and Responsibilities:

  • Assesses the psychosocial status of patients and families/caregivers related to the patient's diagnosis and / or terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required
  • Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed
  • Maintains clinical records on all patients referred to social work
  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs
  • Provides information to patients, families/caregivers, and community agencies
  • Serves as liaison between patients and families/caregivers and community agencies
  • Maintains collaborative relationships with organization personnel to support patient care
  •  
  • Educations and experience required
  • Has one (1) year of social work experience in a health care setting
  • Possesses and maintains current CPR certification
  • Complies with accepted professional standards and practice
  • Maintains current continuing education as required by state licensure: LMSW
