Employment/ Habilitation Specialist

 

POSITION SUMMARY

This position performs duties under the general direction of the Employment Coordinator. The person is responsible for assessing employment skills, developing employment opportunities and providing on-site job training and follow-up services for persons with disabilities. Develop Service plans in consultation with participants and perform requested individualized habilitation services for assigned participants.

Essential Functions Statement(s)

• Participates in the employment services referral process, including gathering information, notifying appropriate parties, and developing employment plans.

• Provides a thorough assessment of Individuals employment needs and interests and skills by using a variety of tools and methods to facilitate the employment process.

• Supports and supervises pre-employment activities which may include such activities as job shadowing, job trials, and benefits counseling.

• Provide all necessary written documentation required by various licensing entities and funding sources.

• Provide on-site training and follow-up services (both to the person employed and the employer) depending on the needs of those served.

• Provide employment counselling as needed.

• Assist and arrange for support of volunteer activities for QUEST participants as requested.

• Represent QUEST in the best possible manner throughout all community transactions to increase awareness around employment options for people with disabilities.

• Document and track work time dedicated to: supporting individuals, travel, and personal staff training.

• Participate in and help to support fund raising activities of the agency.

• Refer participants to community resources for services such as debt counseling, legal aid, housing, and medical treatment or financial assistance.

• Provide individualized supports as requested by participant, family and Supports Coordinator in the community.

• Evaluate personal characteristics and home conditions to provide support in the areas needed.

• Develop and review service plans in consultation with participants and their team.

• Attend required staff meetings.

• Perform other related duties as assigned.

Skills & Abilities

Education: High School diploma - training provided. Preferred Bachelor’s degree, or an Associate’s Degree in a related field.

Experience: None required. Preferred one plus years of experience in Human Services

Computer Skills: Basic computer skills necessary. Knowledge of resume creation and data search engines for job market and community events.

Other Requirements: Knowledge of the Lebanon and surrounding business community. Valid PA Driver’s

License and reliable vehicle available for work use including taking people to search for work and volunteer opportunities.
