JOB SUMMARY:

The Director of Clinical Services and Quality Improvement will oversee and direct all quality improvement activities across multiple departments to ensure that the quality and efficiency of patient care is aligned with HOPE Clinic’s missions, vision, and values. The Director of Clinical Services and Quality Improvement will promote a culture of Continuous Quality Improvement to enhance patient outcomes and advance staff service experience. The Director of Clinical Services and Quality Improvement will be able to collect, analyze, and report clinical data to HOPE Clinic management and the Board of Directors to promote HOPE Clinic’s impact as a community health center.


MAJOR DUTIES & RESPONSIBILITIES:

Clinical Quality Improvements (CQI) and Risk Management:

  • Develop goals and objectives related to Continuous Quality Improvement and Risk Management and its components;
  • Coordinate and oversee HOPE Clinic CQI Plan development, review, revision, and implementation;
  • Chair and manage the CQI monthly committee meetings;
  • Provide monthly written reports to the BOD;
  • Support CMO and CEO in any health service suits and claims;
  • Prepare agendas and all pertinent quality improvement and risk management information to be presented to the CQI Committee, clinic management and the Board of Directors;
  • Coordinate and manage all quality reporting in conjunction with other departmental managers and staff as appropriate;
  • Retrieve, review, analyze, and prepare clinical practice data to present findings to clinic management and the Board of Directors for improved clinical development practices;
  • Provide consultation, counsel and guidance to professional staff within HOPE Clinic;
  • Serve as HOPE Clinic’s Risk Manager;
  • Responsible for the overall direction, coordination, and evaluation of Continuous Quality Improvement and Risk Management program;
  • Develop, coordinate, and administer health center wide systems for risk identification, investigation, and reduction;
  • Maintain statistics to ensure that pertinent patient data relative to incidents is accurate, available and secure;
  • Use evidence-based processes to analyze, recommend and provide support practices seeking to improve performance on quality measures to engage in work redesign, changes in organization systems, policies, procedures and quality improvement process within the organization;
  • Develop, review and revise policies and procedures related to Quality/Risk Management;
  • Review patient/family/physician complaints for liability risk and resolution;
  • Establish screening criteria for review of health center wide patterns;
  • Log and trend incident reports and follow up on problems and/or patterns that emerge from this review;
  • Participate with other departments in improvement activities for successful accrediting, licensing, and certification processes such as PCMH and FTCA applications;
  • Maintain the HOPE Clinic policy manual;
  • Oversee the Quality Improvement Department including areas such as performance improvement and reporting, internal and external audits, data collection, overall quality and compliance management;
  • Perform supervisory responsibilities in accordance with the organization's policies and applicable laws;
  • Interview, hire and train employees within the department;
  • Plan, assign and direct work to achieve the goals as determined by HOPE Clinic;
  • Appraise performance for supervised employees which may include rewarding and disciplining employees;
  • Address complaints and resolve problems across a multidisciplinary team;
  • Supervise the clinical administrative assistant and any future employees in Quality/Risk Management.


Director of Clinical Services

  • Participate in the HOPE Clinic Administrative team meetings, strategic planning and decision-making efforts;
  • Collaborate with the Chief Medical Officer to lead the clinical services of HOPE Clinic;
  • Assist HOPE Clinic Administration and the Chief Medical Officer in new clinical program development;
  • Ensure participatory decision-making processes are followed, and appropriate design and implementation is established;
  • Collaborate with administration, clinic management, and the Chief Medical Officer to set and meet the clinical objectives and goals for clinical care aspects of HOPE Clinic;
  • Participate in the recruitment and hiring of clinical staff and providers;
  • Collaborate with other team members to ensure appropriate clinical care policies and processes are in place to provide high-quality healthcare within the scope of an FQHC;
  • Plan and organize activities related to specified clinical services to ensure patient needs are met in accordance with professional standards of medical care and HOPE Clinic administrative procedures;
  • Coordinate activities and communicate with other HOPE Clinic departments to ensure effective and efficient use of staff, facilities, equipment and other resources while maintaining patient care standards;
  • Supervise the clinical team at HOPE Clinic including medical assistants.


Other Duties:

  • Attend on-site/off-site community engagement activities and clinic events as needed;
  • Perform other duties as assigned to support HOPE Clinic’s Mission, Vision and Values.
Requirements:

QUALIFICATION REQUIREMENTS:

  • 5+ years’ experience working in a community health center required;
  • Knowledge of HIPAA, OSHA, NCQA, HEDIS, and JACHO standards, as well as knowledge of state and federal regulations regarding mandated reporting laws;
  • Excellent customer service skills required;
  • Must possess the ability to communicate effectively with members/customers to assess needs, provide information or assistance, and resolve problems;
  • Must have excellent written and verbal communication skills, the ability to interact with a wide variety of individuals and handle several complex situations simultaneously in a professional manner.


EDUCATION and/or EXPERIENCE:

  • Master’s degree or above in a healthcare related field required;
  • 5+ years of experience working in a community health center required;
  • Previous experience working in an FQHC preferred;
  • Previous supervisory experience preferred.


OTHER SKILLS and ABILITIES:

  • Bilingual (Vietnamese, Chinese, Arabic, and/or Spanish with English) is preferred;
  • Above average skills in language ability as well as public speaking and writing;
  • Must have good transportation and a valid Texas Driver’s license.
