Social Service Coordinator/Community Organizer

The Role:
• Help residents age in place and remain living independently in their own homes
• Advocate for adequate, timely and cost-effective provision of services
• Assess service needs of residents and link them to the appropriate providers and community resources
• Assist residents with applications for benefits and entitlement programs
• Facilitate programs and services such as health and wellness, or transportation
• Develop a resource directory of local social service agencies and providers
• Other related duties as assigned
You’re a great candidate if you:
· Are fluent in English and Spanish (strongly preferred)
· Have 1-3 years of related experience
· Enjoy working with elderly and disabled populations
