Job DescriptionJob Description

We are seeking a compassionate and dedicated Social Worker (LMSW or LCSW) to join our healthcare team. The ideal candidate will provide essential social work services to patients and their families, ensuring holistic care and support. Apply now to join our dynamic healthcare team​​​​​

Responsibilities:

  • Assess nature of clients' situations by interviewing and reviewing personal history'
  • Conduct patient assessments to identify social, emotional, and environmental needs
  • Determine appropriate milestones to gauge client progress
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Assist patients in accessing community resources and support services
  • Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers
  • Maintain accurate medical documentation in compliance with regulatory standards.

Qualifications:

  • Strong interpersonal skills and the ability to work effectively with multidisciplinary teams
  • Compassionate and caring demeanor
  • Ability to build rapport with clients
  • Experience working with diverse patient populations
  • Strong leadership qualities
  • Proficiency in medical documentation
  • Excellent written and verbal communication skills

Requirements:

  • Master's degree in social work from an accredited institution
  • State licensure as a Social Worker (LCSW or LMSW)
