Your role will involve assessing, planning, and implementing rehabilitative programs to address mobility issues, improve functional abilities, and elevate the overall well-being of our participants, empowering them to live fulfilling lives in their communities.

Position Summary:

  • Conducting initial, semi-annual, unscheduled, and annual assessments, participating in daily updates, and reporting changes in participants’ status.
  • Developing and implementing quality assessment and performance improvement (QAPI) activities.
  • Providing regular updates to the team on participants' progress.
  • Assessing needs for adaptive and durable medical equipment (DME), ordering, and facilitating authorization of such equipment.
  • Participating in various meetings (morning, team, family, nursing facility, assisted living facility, and hospital) to ensure comprehensive care.
  • Delivering indicated treatments and utilizing appropriate safety measures.
  • Creating special programs tailored to participant needs and educating staff, participants, and families about available services.
  • Supervising and training physical therapist assistants and restorative aides.
  • Reviewing and revising restorative exercises as needed.
  • Conducting orientation and training on body mechanics and lift equipment.
  • Completing required documentation and maintaining confidentiality.
  • Adjusting schedules to meet participant needs and providing home therapy services when necessary.

Requirements:

  • At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).
  • A Bachelor's degree in Physical Therapy from an accredited program and a current California license.
  • Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.
  • The ability to cope with mental and emotional stress, work independently, and collaborate effectively with team members.
  • Excellent communication skills, organization, dependability, flexibility, and resourcefulness.
  • A valid California driver's license with a good driving record (for home visits).
