We are seeking a dedicated and compassionate Physical Therapist to join our team at HealthCare Talent. As a Physical Therapist, you will play a vital role in enhancing the lives of elderly participants by delivering top-notch physical therapy services.

**Key Responsibilities:**

  • Conduct initial, semi-annual, unscheduled, and annual assessments, participating in daily updates, and reporting changes in participants' status.
  • Develop and implement quality assessment and performance improvement (QAPI) activities.
  • Provide regular updates to the team on participants' progress.
  • Assess needs for adaptive and durable medical equipment (DME), ordering, and facilitating authorization of such equipment.
  • Participate in various meetings to ensure comprehensive care.
  • Deliver indicated treatments and utilize appropriate safety measures.
  • Create special programs tailored to participant needs and educate staff, participants, and families about available services.
  • Supervise and train physical therapist assistants and restorative aides.
  • Review and revise restorative exercises as needed.
  • Conduct orientation and training on body mechanics and lift equipment.
  • Complete required documentation and maintain confidentiality.
  • Adjust schedules to meet participant needs and provide home therapy services when necessary.

**Requirements:**

  • At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).
  • A Bachelor's degree in Physical Therapy from an accredited program and a current California license.
  • Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.
  • The ability to cope with mental and emotional stress, work independently, and collaborate effectively with team members.
  • Excellent communication skills, organization, dependability, flexibility, and resourcefulness.
  • A valid California driver's license with a good driving record (for home visits).
