JOB DESCRIPTION

POSITION TITLE: Community Health Response Worker

REPORTS TO: Community Health Response Supervisor

SUMMARY: Individual will conduct in person and telephone conversations with individuals providing prevention education, general health and wellness education, and conduct community engagement, along with identifying available healthcare and social resources to community members.

DUTIES AND RESPONSIBILITIES

  1. Adheres to the mission of Central States SER
  2. Coordinating resource navigation efforts with Supervisors and The Chicago Cook Workforce Partnership whom receives direction from Chicago Department of Health
  3. Conducting interviews in a professional manner that reflects emotional and cultural awareness
  4. Collecting and recording accurate information using Customer Relationship Management (CRM) software such as; NowPow, Nice and Contact, Career Connect and Microsoft Teams.
  5. Interviewing individuals and utilizing assessments make recommendation of resources available, along with disseminating health information
  6. Providing contacts with regulated information on isolation and quarantine procedures as directed
  7. Collaborating with designated local authorities in supporting public health initiatives
  8. Facilitating health checks, as well as referring contacts to available community resources
  9. Participate and conduct in person community outreach events to educate and inform individuals on various topics related to public health.
  10. Complying to all Health Department regulations, including those aimed at protecting personal information
  11. Protect and maintain individuals’ privacy and confidentiality
  12. Performs other related duties as assigned

JOB REQUIREMENTS:

  1. Must be able to work in office and remotely
  2. High School Diploma or GED required but College-level education preferred
  3. Reside in a Priority Community Area as identified by The Chicago Department of Public Health such as North or South Lawndale
  4. Proficiency in utilizing Windows Operating system and performing data entry; previous experience in CRM software is an additional benefit
  5. Advanced proficiency in English required, bilingual Spanish proficiency is preferred
  6. Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
  7. Ability to comply with regulations and communicate official information in an appropriate manner
  8. Advanced ability to coordinate efforts, facilitate intervention, and maintain records
  9. Exceptional organizational skills and sound judgment
  10. Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds
