Summary:

The Assistant ADC LPN is responsible for supporting the overall daily operations of the Adult Day Care Facility in Green Cove Springs. They will report directly to the Adult Day Care Program Supervisor and will provide supplemental supervisory oversight of the CNA’s. They will also be responsible for overseeing daily client care to ensure all clients receive exceptional care while spending time in our day care.

Qualifications/Requirements:

  • Currently licensed as a Licensed Practical Nurse in the State of Florida.
  • Current CPR Card through American Heart Association or American Red Cross W/ Hands on Demonstration.
  • Must submit to DOEA/AHCA background checks and drug screening.
  • Must have strong written and oral communication skills.
  • Must possess a commitment to helping seniors.

Essential Functions:

  • Oversees all aspects of client care for the Green Cove Springs ADC Facility’s day to day operations in conjunction with the ADC Program Supervisor.
  • Will become acting shift supervisor when ADC Program Supervisor is away from the facility.
  • Collaborates care with the assigned C.N. As at the Green Cove Springs facility.
  • Assists with maintaining the daily and weekly census for the ADC client attendance.
  • Assists with the maintenance of all client charts as required by the State or Veterans Administration’s guidelines.
  • Assists with the maintains all paperwork for the Green Cove Springs facility including, but not limited to, initial intake assessments, admission packets, 3008 forms, DRN statuses, and emergency contact information for both clients and staff.
  • Administers all medications and treatments according to MARs.
  • Maintains and updates the Medication Administration Records (MAR) for each client.
  • Assists with the creation and implementation of monthly Activity Calendars for ADC Locations.
  • Maintains monthly Meal and Snack Calendars for distribution to families, at ADC Locations.
  • Provide personal care assistance in addition to the CNAs for clients at the ADC to help with maintaining clean and proper hygiene throughout the daily activities.
  • Oversees the implementation and upkeep for the Activities of Daily Living (ADL) Tracking Sheets.
  • Utilizes good judgment and decision-making skills in the delivery of patient care.
  • Communicates effectively with patients, family and health care providers regarding clients.
  • Performs other nursing related duties as requested.

Working Conditions:

  • Business hours are Monday through Friday, 8:00 am-5:00 pm
  • Due to circumstances that may arise, overtime maybe required at times.
