Job DescriptionJob Description

The Harbor Senior Care Manager is responsible for overseeing all aspects of the Memory Care in order to have efficient functioning of day-to-day operations including:

1. Develops a therapeutic environment for Residents with Alzheimer’s and other forms of Dementia while keeping in accordance with established policies and procedures and state and federal regulations.

-Plans and organizes daily program to promote engagement and stimulation

-Leads daily Caregivers

-Assures that Resident Rights are respected.

-Mandates staff to abide by strict standards to assure Resident confidentiality.

-Maintains a safe, secure and clean home.

2. Assists with providing staff with appropriate training and support to address the emphasis of the program.

-Recruits, screens and interviews new personnel

-Assures staff completes orientation and annual training in Care Academy.

-Provides support, direction, feedback, and training for staff members when necessary.

-Delegates responsibilities to staff with clear definition of expectations.

-Ensures that all employees know policies and procedures, job duties, safety and emergency procedures

-Schedules, hires, disciplines/terminates, and supervises Memory Care staff.

-Evaluates the performance of each employee periodically

-Provides opportunities and materials for regular staff meetings and in-service education

3. Provides quality care for residents and maintains relationships with Residents, their family, and the community.

-Assures that each Resident and their families are treated with respect and dignity.

-Follows individualized service plans that were implemented by Director of Nursing

-Communicates changes in resident’s behavior or health with family and Director of Nursin

-Proactive about expectations and follow through, with all staff, regarding safety in every area.

4. Coordinates staffing and responsibilities to meet the needs of the residents as a whole and assures quality of services through continuous Quality Assurance.

-Responsible for schedule using our platform “Makeshift” and maintaining staffing in order to provide safe staffing ratios, depending on census and acuity.

-Ensure adequate staff to meet the needs of residents on a 24 hour basis.

-Maintains schedule 6 weeks out at all times.

-Regularly communicates with staff via “Group me” or “The Harbor Senior Care Crew” or via text or phone call.

-Improves the program through problem solving, program development and refinement.

-Responsible to follow up on certifications or licenses to ensure up-to-date and not lapsed certifications.

5. Maintains program with financial guidelines

-Assures that the community operates in line with the established budget and meets income objectives

-Maintains budget and meets with Administrator weekly to review.

-Assures Resident and family satisfaction as a means to develop family source of referrals.

-Actibavely participates in marketing throughout community by attending events and socials.

6. Maintain program with Grove Menus for Meal planning.

-Print out menus and food prep on a weekly basis.

-Assure Resident diets are followed closely and advise Director of nursing of any needed. changes.

-Delegate staff to order groceries through HYVEE or Sam’s Club.

6. Provides an atmosphere to assure staff satisfaction

-Addresses staff concerns and questions in a timely manner

-Uses problem solving skills and creativity to provide a good work environment in coordination with the Director of Nursing.

-Serves as a team player and leads through example.

Miscellaneous responsibilities:

-Order items needed to keep the home functioning properly.

-Maintain Lawn and snow removal services

-Maintain Insect/Bug services

-Maintain Housekeeping services

-Maintain annual fire alarm/fire extinguisher services

-Maintain monthly house inspections
