Job DescriptionJob Description

We are currently looking for Support Coordinators to support individuals with disabilities in Bergen, Hudson Passaic Middlesex, Monmouth Morris, and Union County. The successful candidate will be responsible for assisting participants to gain access to needed programs, services and supports including but not limited to medical, social, educational, and other services.

Responsibilities and Duties:

· Assist individuals with developmental disabilities in finding services and supports in their community.

· Draft and revise Individualized Service Plans that focus on the individual’s goals and aspirations

· Find service providers in the community to meet an individual’s needs.

· Check to make sure the services are meeting an individual’s needs monthly.

· Conduct quarterly and annual visits.

· Answer questions on service providers and community involvement

· Respond to emergencies.

Qualifications and Skills:

· Bachelor’s Degree (in any field) REQUIRED.

· Minimum of 1 year experience working with individuals with disabilities.

· All employees are required to be fully vaccinated against COVID-19. Including Booter. 

· Able to pass Background Check and Cari Check.

· Great communication skills (oral and written).

· Computer proficient.

· Must be a valid licensed driver and have a personal vehicle that is registered and insured.

· Must possess a New Jersey driver’s license with 5 points or less.
