Substance Use Assessment Counselor
Full Time - Exempt
1. General Description
Meets with clients and completes all admission documentation for residential treatment setting including biopsychosocial assessment, initial assessment, clinical assessment, and client treatment plan. Communicates with the assigned counselor all pertinent information regarding client intake information and process. Also completes the Adult Needs and Strengths Assessment for the Division of Mental Health and Addiction and enters the information into the Division of Mental Health and Addiction data system. Prepares all documentation for Medicaid billing for 3.1 Clinically Managed Low Intensity Residential services.
2. Reporting Relationship
The Substance Use Assessment Counselor reports to the Vice President Programs and Services.
3. Supervises
The Substance Use Assessment Counselor has no supervisory responsibilities.
4. Responsibilities and Essential Functions
· Completes all admission documentation for funding sources, including Medicaid, Division of Mental Health and Addiction, Department of Child Services, Recovery Works and other funding streams.
· Meets individually with clients to complete admission documentation, biopsychosocial assessment, clinical ASAM assessment, intake assessment and person-centered treatment plan.
· Schedules intakes for the substance use treatment program and communicates this information to the appropriate counselor and the Vice President of Programs and Services.
· Encourages client’s family to be involved in treatment, if client so chooses.
· Completes all Medicaid documentation for client substance use treatment including Prior Authorization, Initial Assessment, Clinical ASAM Assessment, Biopsychosocial, and Treatment Plan and submits for Medicaid billing.
· Ensures that treatment counselors are aware of Medicaid reassessment dates in a timely manner.
· Becomes certified to complete Adult Strengths and Needs Assessments in the DARMHA system.
· Becomes and maintains Super User status in the DARMHA system.
· Completes the Division of Mental Health and Addiction Enrollment, Assessment, Substance Use Disorder Treatment and Discharges into the DARMHA system.
· Prepares client chart and documents all pertinent information in the chart to pass on to treatment counselor following initial admission to substance use programming.
· Communicates with treatment counselors and Vice President Program and Services any information that may affect a client’s treatment at the YWCA, such as needing a higher level of care for mental health or medical health than can be provided at the YWCA.
· Completes substance use assessments to determine the eligibility of potential clients.
· Communicates with jails and prisons in Indiana and maintains a jail waitlist for substance use services.
· Facilitates Substance Use Treatment group, Dual Treatment group & Aftercare in counselor’s absence.
· Compiles and submits documentation for requests of client’s treatment history at the YWCA when requested for former clients by Disability Determination Bureau, Department of Child Services, probation, parole, drug court, attorneys, and other entities.
· Participates in multi-disciplinary team staffing, team meetings, case consulting, community activities and organizations; prepares and delivers appropriate lectures to the public as requested by the Vice President Programs and Services.
· Participates in planning and evaluation session for continued growth of the program; works as part of a team in assuring quality delivery of services to YWCA clients.
· Ensures appropriate and accurate documentation regarding client interactions, in compliance with
agency and grant standards.

· Performs other duties as assigned by Vice President Programs and Services.
5. Education and Experience
Master’s degree in counseling, social work or related field; ICAADA certified or certifiable; LSW, LCSW, LMHCA, or LMHC preferred with two to three years related experience, or an equivalent combination of training and experience; requires certification or completion of CPR training course, updated yearly
6. Knowledge and Ability
Requires an in-depth knowledge of the disease concept of substance use, individual, group and family system counseling skills and an understanding of the treatment and recovery process of the AA/NA program; requires demonstrated ability to make independent decisions regarding client admission, course of treatment and discharge planning and recommendations; required demonstrated interpersonal skills to communicate in a responsible, empathetic and effective manner with participants and to work effectively with a multidisciplinary framework; attention to detail, as well as skilled time-management and organization required; personal experience with the disease of chemical dependency is helpful, either as a member of family recovering from the illness or as a chemically dependent person; individuals with a history of chemical dependence must have a two-year period of chemically free living and are required to maintain continued sobriety during the term of employment.
