POSITION SUMMARY

Looking for a Full Time Case Manager for 2024

Under the supervision of the Housing Manager, the Case Manager is responsible for providing advocacy and case management services for the clients of Coram Deo Recovery. In an effort to foster stability and self-sufficiency during the client’s recovery, the Case Manager develops an individualized recovery plan based on identified needs, facilitates access to social services, monitors the client’s progress and advocates on behalf of the client, as appropriate.

PRIMARY DUTIES AND RESPONSIBILITIES

  • Address immediate (primary needs) such as food, clothing and safety so the client can focus on recovery goals.
  • Engage the client and client’s support systems to gather, evaluate, and integrate pertinent information to complete a comprehensive needs assessment. Allow the client to take the lead in determining what’s best for her.
  • Develop a thoughtful, time limited, and measurable individualized recovery plan in collaboration with the client and based on assessment findings. Establish common goals
  • Review, update and modify recovery plan as needed with the client, other professionals, and the client’s natural supports. and encourage shared problem solving.
  • Develop and facilitate partnerships through collaboration with professionals from different disciplines as necessary to provide wrap around services for the client.
  • Research, maintain, and share information on community and other resources relevant to the needs of clients.
  • Comply with all Coram Deo Recovery data entry, integrity, tracking, and reporting requirements. Includes completing written notes in the case management packet after meeting with client each week. Also input notes into the state ABH system (ATRIV or BHRP-RSP system).
  • As appropriate, coordinate daily transportation needs of clients directly or by instructing in use of public transportation, tapping into other transportation sources, or coordinating with other team members.
  • Attend residential staff meetings as appropriate.

QUALIFICATIONS The requirements to perform the essential duties and responsibilities of the Case Manager position are outlined below.

Experience & Education

  • At least 3 years of experience working with substance abuse clients and related issues, including co-occurring disorders
  • BA degree in social work, psychology or related human services field is preferred.

Other Requirements

  • Bilingual or multi-lingual skills appropriate to the client population served are preferred.
  • Valid CT driver's license and the regular use of your car (mileage is paid) are required.
  • Requisite Competencies Addiction/Recovery Management: Demonstrated knowledge of addiction and recovery management techniques/models along with community, social, and health resources, particularly substance abuse, mental illness, and addiction-related resources.
  • Recovery Planning: Skilled in performing needs assessments and developing individualized recovery plans. As part of assessment process, proficient in gathering and reviewing information through a holistic approach.
  • Mentoring and Coaching: Proven mentoring and coaching skills. Helps clients see any discrepancies between present behavior and planned goals.
  • Relationship Management: Demonstrated expertise in building working relationships with clients, their family, professionals, and the community, including the ability to collaborate with a multidisciplinary health care team.
  • Cultural Competence: Demonstrated sensitivity to ethnic, cultural, gender, and sexual orientation diversity encompassing values, beliefs, and behaviors.
  • Communication: Effective verbal and written communication skills in facilitating interactions with clients, their families, and professionals. Includes proficiency in recovery management interviewing skills such as active listening, motivational interviewing, supportive responses, open- and closed-ended questions, summarizing, and giving options.
  • Organizational Skills: Strong multi-tasking and prioritization skills in addressing potential obstacles such as managing a caseload, limited resources, clients who stop engaging, maintaining cultural competence, and keeping current on documentation.
SUPERVISORY RESPONSIBILITIES

This position may supervise Volunteers and Interns.

WORK ENVIRONMENT

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. This position entails frequenting various community places. These may include peoples’ homes, work sites, shelters and other outside areas.

Salary: $15.00 - $22.00 per hour
