We are seeking a care coordinator to join our team.
• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centered.
• Developing and maintaining a good working relationship with all referral sources and preparing weekly, monthly reports.
• Handle case assignments, review case progress at all times.
• Facilitate multiple care aspects (case coordination, information sharing, and providing updates to external case managers.
• Manage all patient processes to keep track of their progress and to ensure their satisfaction.
• Maintain case records, maintain accuracy of all forms, documents and produce statistical reports
• Promote quality and cost-effective interventions and outcomes.
• Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
• Keep up to date on department and company specific initiatives and timelines.
• Complying with health care regulations and ensuring patient confidentiality
• Maintain orderly and up to date files for all clients and caregivers.
• Manage the timely clock in and out of caregivers and timely submission of their timesheets.
