The Outreach Worker is a critical member of the Supportive Services team of Lifting Up Westchester (LUW) who identifies and engages with former and currently homeless individuals through the LUW soup kitchen and street outreach, as well as at-risk individuals served by other providers. The Outreach Worker assists clients with securing benefits from the Social Security Administration and Department of Social Services (DSS), as well as assisting with them with obtaining homeless shelter placement and needed services from other agencies.

  • Perform outreach to the homeless individuals and the at risk community through the LUW soup kitchen, street outreach, and networking with other agencies to identify potential clients.
  • Perform one on one client interviews to evaluate client needs and complete program intake.
  • Through on-going meetings with clients, establish client goals and evaluate resources needed to attain each goal.
  • Make appropriate referrals to other agencies to obtain needed services.
  • Coordinate with various entities including the Department of Social Services, Social Security Administration, Department of Vital Records, and Department of Motor Vehicle to assist clients in obtaining official identification documents, benefits, and entitlements.
  • Complete all applications and paperwork required for clients to receive referred services, and support the client through the process of securing these services.
  • Advocate for the client with other providers to create a pathway for the client’s ongoing care.
  • Create and maintain client files
  • Maintain client database for tracking outcomes and update client records in a timely way.

QUALIFICATIONS:

  • Two years of college education. Bachelor's degree helpful.
  • 3 - 5 years’ experience working in providing direct services with the street/sheltered homeless population.
  • Knowledge of the Westchester County Department of Social Services’ Homeless Services Department.
  • Microsoft Word, Excel, and email proficiency.
  • Must be able to assess client needs and make appropriate referrals.
  • Candidate must be a self-starter and have strong organizational skills.
  • Must have a Valid Driver’s license.
  • PHYSICAL REQUIREMENTS: This position requires community outreach and will require considerable time outside of the office standing and walking, as well as advocating for clients in meetings with other service providers.
