Job DescriptionJob Description

Responsibilities:

  • Handles intake for prospective clients to determine service needs. Conducts home assessments and completes start of service documentation and onboarding for prospective clients.
  • Initiates the plan of care and makes necessary revisions as client status and needs change in collaboration with care staff and other healthcare professionals
  • Coordinates caregiver selection and scheduling with office staff to achieve optimal client-caregiver compatibility.
  • Conducts pre-assignment training and communication with caregivers to prepare them for new assignments or changes in condition. Assists with caretaker recruiting and training as needed to meet agency staffing goals.
  • Conducts regular home visits as assigned, assessing the client's condition and caregiver in-home performance. May communicate with community health related persons to coordinate the care plan. Ensures that arrangements for equipment and other necessary items and services are available and communicated to the client and family.
  • Manages the VA Community Care Network referral program from intake to start of service to ensure efficient onboarding of VA clients. Builds relationships with VA case managers.
  • Provides instruction, supervision and evaluation of caretaker performance and works with operations to maintain optimal quality standard. Actively participates in quality assessment performance improvement teams and activities
  • Acts as the community liaison by promoting agency services to the public, case managers, other healthcare providers and associations. Serves as the agency representative by advocating for the agency and building relationships that create a referral network. Activities may include in services, presentations, health fairs, etc.
  • Provides supervision and mentoring to office staff that fosters agency growth, client and caretaker acquisition and retention.

Requirements:

  • Prior healthcare case management experience, preferably in a home care setting.
  • Demonstrated leadership and training skills and experience.
  • Experience recruiting employees in home care or home health care preferred.
  • Desire to work with employees and clients of all ages with a passion for serving the senior population.
  • Exceptional communication, organization and time management skills.
  • Attention to detail and the ability to prioritize tasks and meet deadlines.
  • Comfortable working in a dynamic environment where client and employee needs may change frequently.
  • Flexible schedule with the ability to travel to client homes in the service area.
  • Must reside in the Boerne area or can easily commute to the office location.
  • Punctuality and attendance are critical for the position.
  • Bilingual (Spanish) preferred.
