Pay Rate: $45-$50 per in-person assessment, $35-$40 per virtual assessment

Job Description:

Sweet Little Helpers Home Care Agency is seeking a compassionate and experienced Registered Nurse to perform both in-person and virtual assessments for our home care clients. As a key part of our care team, you will assess clients’ medical and personal care needs to develop customized care plans. 

This position requires an initial in-person assessment followed by alternating monthly assessments, which will alternate between in-person and virtual visits. Compensation will be based on the type of visit.

Responsibilities:

- Initial In-Person Assessment:  
  - Conduct a thorough evaluation of the client's health, functional ability, and environment.
  - Assess medical conditions, medications, and any assistance required with daily living activities.
  - Document findings, create a detailed care plan, and provide recommendations for ongoing care.
  - Communicate with the client's family and care team to ensure understanding of the care plan.

- Second (Virtual) Assessment:  
  - Follow up with the client virtually to ensure the initial care plan is being followed.
  - Address any changes in the client's condition, update the care plan as needed.
  - Provide guidance to the caregiver team based on the client’s current needs.
  
- Ongoing Monthly Assessments:  
  - Alternate between in-person and virtual visits each month.
  - Monitor the client’s progress, reassess the care plan, and make updates if necessary.
  - Document changes in the client’s health or functional ability and communicate them to the care team.

Qualifications:

- Valid Registered Nurse (RN) license in the state of New Jersey.
- At least 2 years of nursing experience, preferably in home health care or geriatrics.
- Strong communication and documentation skills.
- Ability to conduct virtual assessments and manage telehealth technology.
- Must have reliable transportation for in-person visits.

Compensation:

- In-Person Assessments: $45-$50 per visit.
- Virtual Assessments: $35-$40 per visit.
- Travel reimbursement may be available depending on the location of the client.

Schedule:
This position allows for flexible scheduling based on client needs and your availability. The initial assessment must be completed in person, while follow-up assessments will alternate between in-person and virtual visits.
