Summary

The Case Manager supports Baltimore residents from historically marginalized communities in building family-sustaining green careers. They will coordinate, facilitate, and provide case management services to all participants enrolled in Civic Works’ Center for Sustainable Careers (CSC) program.

CSC serves adults and out-of-school young adults from historically marginalized communities, who often face systemic barriers to employment. This includes residents who have been impacted by the criminal justice system, have experienced homelessness, have insufficient educational attainment, are recovering from addiction, have inadequate financial stability, and lack access to reliable transportation.

 Primary Responsibilities

  1. Provide comprehensive case management services to program participants, including intake assessment, short and long-term case plan development. Specific duties include providing the following case management services: 

    1. One-on-one meetings with program participants to assess their needs. 

    2. Direct support, guidance, and advocacy for program participants working to meet their goals, including but not limited to:

      • Obtaining a valid driver's license and vehicle

      • Addressing participants’ legal concerns, such as open traffic, criminal, or civil cases; or meeting supervision requirements within the criminal justice system

      • Meeting basic needs for housing, food, healthcare, or clothing

      • Developing financial stability

      • Helping participants develop resilience, healthy coping skills, positive social skills, and workplace readiness

      • Assisting in the administering of random drug screenings

    3. Group psychoeducational therapy, including, but not limited to, harm reduction, parenting, budgeting, and anger management.  

      • Referrals to other organizations providing program participants with information and services in areas such as housing, financial, education, medical, and counseling. Tracking and supporting program participants’ ISP progress throughout enrollment and at a minimum of twenty-four (24) consecutive months immediately following participant’s exit date. Meeting regularly with program staff to report each participant’s ISP case status.Maintains program participant’s case file by collecting, verifying and updating relevant information.

  2. Documents all program interactions with participants, including writing daily case notes.

  3. At the direction of the Director of CSC, creates and submits case notes and reports in accordance with grant requirements as needed and appropriate. 

  4. Advocates for program participants as needed and provides follow-up on all program participants. 

 

Secondary Responsibilities:

  1. Works with the Training Team to ensure participant feedback is included in planning for future outreach and program growth.

  2. Conducts periodic reviews and audits of records to ensure an active, accurate program information system based on proper service control/standards, and regulatory agencies. 

  3. Participates in staff and team meetings. Communicates and coordinates with the Student Services Manager and appropriate staff as needed. 

 

EDUCATION AND EXPERIENCE:

  1. Experience working in and creating diverse, equitable, and inclusive environments.

  2. A deep understanding of the multiple forms of systemic and racialized inequities that prevent people from accessing and retaining employment.

  3. Minimum of one (1) year case management or equivalent experience in counseling, coaching or mentoring.

  4. Strong computer and note-taking skills required. Experience with Microsoft Word and Excel programs required.

  5. Knowledge with community resources and referral processes, particularly in an urban context.

  1. Ability to establish and maintain effective counseling relationships with program participants. 

  2. Ability to engage in self-care and prioritize healthy workplace boundaries.

  3. Highly organized, detail-oriented, and able to manage multiple projects simultaneously.

  4. Ability to work independently under pressure and tight deadlines. 

  5. Excellent verbal communication skills, interpersonal relations skills, and writing skills.

  6. Ability to recognize problems, develop innovative solutions and implement them

  7. Ability to adhere to a strict guideline of keeping and maintaining confidential information. 

  8. Ability to prepare and submit accurate and reliable reports. 

  9. Able to build deep supporting relationships with program graduates from historically marginalized communities.

  10. Committed to advancing racial equity across systems in Baltimore.

  11. Must have a valid driver's license and transportation to commute to the office.
