Job DescriptionJob Description

Position Objective The Medical Coordinator is responsible for overseeing all medical support for people who receive residential services at EPC. This person is responsible for ensuring that EPC is in compliance with Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, Rule 140, and other relevant regulatory guidelines. This person is responsible for the direct supervision of the Health Services Direct Support Professionals. This person will also ensure that all medical supports for people follow best practice and works with the interdisciplinary team to ensure people have the best possible health.

Essential Duties/Responsibilities

Mission, Vision, and Values

  2. Manage all responsibilities based on the agency mission, using individual choice and all
  3. appropriate policies, procedures, and regulations.
  4. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.
  5. Perform all safety and emergency procedures in a calm, accurate and professional manner
  6. Follow and implement all pertinent agency policies, departmental protocols, and procedures.

Assistance to Director of Nursing

  1. Satisfactorily complete IDHS Medication Administration training.
  2. Complete annual training refresher based on EPC policy and state regulations.
  3. Entering lab orders in AHA portal for lab draws, printing requisition forms for the phlebotomist, and entering t-logs for any pre-lab requirements, such as but not limited to fasting, trough levels, specimen collections, etc.
  4. Maintaining a clean and safe working environment, including stocking the medication cart with supplies, cleaning and disinfecting the medication cart and work station, and coordinating transportation of medications between CDS and residential sites.
  5. Daily medication shipment check-in, notifying the appropriate RN Trainer of new medications received, and notifying appropriate house manager/host home provider for delivery of medications to residential sites.
  6. Conducts weekly in home medication audits to assure medications systems are in place.
  7. Attends team meetings at needed for all medical concerns. This may be person specific or site specific.
  8. Attends critical medical appointments to assist team in accurately communicating concerns for people supported.
  9. Works with medical providers to ensure orders are received for all routine medications and prn medications
  10. Mentors the team on ensuring all follow-up is completed for ER visits, hospitalizations, urgent care visits, etc.
  11. Supports the team in finding resources for all medical needs within each person’s specific insurance.

Clerical Duties

  1. Complete and submit required documentation after providing support to include but not limited to T-Log, case note, tracking, GER, and email updates to team members.
  2. Obtain the needed paperwork following all medical appointments to share with nursing and file in the person’s chart.
  3. Complete needed steps to ensure that each person’s chart and file is up to date and accurate.
  4. Assist in the assessment of resource requirements, including, staffing, equipment, and supplies.
  5. As needed, schedule auxiliary medical services to include but not limited to internal primary physician, dental, podiatry, and vision clinics.
  6. Ensure documentation and records are maintained in accordance with pertinent policies, procedures, and standards of practice.
  7. Provide required documentation included in the hard and electronic files for people supported.
  8. Ensure documentation is completed daily for all duties including medical, behavioral health and programmatic goals.
  9. Coordinates and/or conducts clinics, lab draws and other specialties as needed for people supported to receive the medical attention they need.
  10. Assures regular screenings and assessments are completed as required for each person.
  11. Acts as main contact for external stakeholders to assure all nursing needs are met as required by Rule 115, Rule 116, and Rule 119.
  12. Ongoing communication with Pharmacy to ensure all medication orders occur and there are no gaps in the delivery of medications to people supported.
  13. Revises and monitors tracking system for all residential person-specific medical needs, and assures 100% departmental compliance per Rule 115/116.

Medical Coordination

  1. Easily recognizes when there is a gap in medical services or supports for people and works with Director of Service Coordination, QIDPs and appropriate team members to address
  2. Continuously seeks feedback to ensure that medical supports are meeting people’s needs. This includes but is not limited to clinics and community providers.
  3. Recognizes and highlights unmet critical service needs and works with the team and the other key stakeholders to proactively address identified concerns.
  4. Maintains current partnerships with current providers
  5. Recognizes areas for additional partnerships and seeks those partnerships out in order to bridge any existing service gaps.
  6. Works with departmental teams to ensure compliance with CMS standards as well as BALC, BQM, OIG, DCFS, CQL etc. are consistently met.

Person-Centered Support

  1. Involve the person supported in their appointments and care in accordance with the principles of age-appropriateness and normalization.
  2. Assure dignity, privacy and rights of program participants are always observed
  3. Assure that the basic civil, human, and legal rights of all persons are respected.
  4. Schedule and transport people supported to medical and other related appointments/services.
  5. Ensure that all needed information is shared and presented at medical appointments.
  6. Work cooperatively to link the department with services and resources that have the potential to improve the delivery of health services.
  7. Recognizes individual differences and adapts teaching/ interacting style to best support the person's needs.
  8. Advocates for individuals through knowledge of personal references, protection from abuse and neglect or other rights violations, and supporting the development of personal relationships.
  9. When necessary, assist individuals with personal needs to include but not limited to transferring, toileting, and hygiene needs.
  10. Delivers services and supports as outlined in everyone’s Person-Centered Plan ensuring that activities and supports reflect individual preferences.
  11. Assist the individuals in striving toward independence in accordance with goals and objectives in the Personal Plan on the individualized Service Plan.
  12. Act as a positive role model for individuals at all times.
  13. Provide natural learning opportunities to teach independent living skills in accordance with the principles of normalization.

Marginal Responsibilities

  1. Aid nursing in scheduling and transporting people supported to medical and other related appointments/services. These may include, but not limited to counseling sessions, pre-operative tests and consultations, health care assessments and evaluations, dental care, and emergency care.
  2. As directed, communicate verbally and in writing the recommendations to nursing staff.
  3. Will also assist with clerical support such as filing, copying, and data entry.
  4. Perform other duties as assigned.
  5. As a mandated reporter, ensures all allegations abuse and neglect are reported as required.

Education/Experience Requirements

  1. Must be at least 18 years of age.
  2. High School Diploma or equivalent (GED).
  3. Licensed Practical Nurse, Certified Medical Assistant or Medical Assistant required.
  4. Successfully complete CPR, First Aid, Infection Control, and Crisis Prevention Institute (CPI) training and maintain these certifications.
  5. Must complete DSP training in accordance with the state regulations and maintain acceptable status on the Health Care Worker’s Registry under the Healthcare Worker’s Background Check Act.
  6. Must obtain/maintain standing as an Authorized Direct Support Person for medication administration.
  7. Possess a valid driver’s license and automobile insurance.
  8. Have access to an automobile
  9. Have access to a telephone
  10. Must be able to perform CPR/First Aid
  11. Must be able to pass the criminal background check.

Physical Requirements

  1. Frequent walking and sitting.
  2. Regular reaching, grasping, and standing.
  3. Must be able to assist in evacuating residents in emergencies.
  4. Other physical activities related to an office environment.
  5. Operates general office equipment, including multi-line phone, fax machine, printer, copier, and personal computer.
  6. Ability to lift 50 lbs.

  1. Work may occur in the following settings: Office, classroom, site, group homes, medical office, hospital, or community.
