Position: Full-Time Registered Nurse

We are dedicated to providing exceptional care both in our clients' homes and at our adult day care facility. We are seeking a compassionate and dedicated Registered Nurse to join our team.

Job Description:

As a Registered Nurse at SarahCare, you will have the opportunity to work in a dynamic and supportive environment. Your responsibilities will be divided between our adult day care facility and home care services.

Home Care Responsibilities:

  • Conduct assessments at clients' homes.
  • Review and sign off on care plans.
  • Ensure the delivery of high-quality care in accordance with company policies and state regulations.
  • Provide guidance and support to home care aides and other staff members.
  • Communicate effectively with clients, their families, and other healthcare professionals to coordinate care.

Day Care Responsibilities:

  • Conduct patient assessments and monitor vital signs.
  • Develop and update care plans for day care participants.
  • Administer medications and treatments as needed.
  • Maintain accurate and timely documentation and notes.
  • Collaborate with other healthcare professionals and staff to provide comprehensive care.
  • Ensure a safe and welcoming environment for all participants.

Qualifications:

  • Current Registered Nurse (RN) license in the state of Pennsylvania.
  • Minimum of 2-3 years of nursing experience, with hospital or geriatric experience required.
  • Strong assessment and clinical skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and patient-centered approach to care.
