LOCATION: Largo MD, or Lanham, MD


Summary:

The position provides coordination and oversight of the case management and PCP functions for all DDA community living services, to ensure a consistent person-centered planning process throughout all programming. Duties include establishing and monitoring a master schedule of PCP and quarterly review dates, and working closely with internal staff and Coordinators of Community Services (CCS) to ensure all quality standards are met. Assists in accepting and/or declining services in LTSS. The CPCP is responsible for creation, maintenance and training on any and all PCP documents to facilitate successful annual and interim meetings. The person must be efficient, detail oriented, process driven and ensure all necessary information is communicated appropriately and facilitate meetings. This position is also responsible for training on and compliance with PCPs requirements.


Responsibilities:

Manages the implementation of individual supports consistent with the most current DDA requirements; Fosters collaboration with OHCQ, CCS agencies, internal staff and community partners on an ongoing basis. Oversight and management of service delivery to ensure quality and meaningful active treatment. Responsibilities encompass a broad range of activities including developing and/or encouraging the use of best practices in program development and person centered planning practices.


Programmatic Duties:

· Oversee the process of ensuring proper preparation of all PCP documents and activities by each Community Living program, to guarantee a successful and efficient PCP meeting.

· Serves as an advocate for all individuals supported in the Arc’s Community Living Programs to ensure their wishes and dreams are accurately represented in annual plans.

· Train staff and oversee timely submission of Behavioral Support Plans (BSP’s)

· Train Community Living staff and oversee proper documentation of Rights Information to the HRAC.

· By random and scheduled reviews, ensures that all services on the PCP are aligned with current waiver services.

· By Submit all necessary paperwork for funding (DSAT or SSP) and planning (PCP) to DDA.

· Ensure that all necessary services are funded and to ensure that all services that are delivered are identified as funded by DDA correspondence.

· Design and implement support and teaching strategies targeted at assisting clients with reaching their outcomes, as outlined in the client's PCP

· Provide supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies

· Collaborate with the Director of Admissions on accurate PCP and supporting documentation for the annual plan as well as Nursing supports.

· Evaluate and revise support and teaching strategies, as necessary, to ensure progress toward goals.

· Ability to work effectively with direct care, clinical, professional and administrative level staff and to interact in a professional and therapeutic manner with families or community agencies.

· Participate in highly sensitive and critical Person Centered Plan meetings

· Other projects and duties as assigned.

Requirements:

Asset Management:

1. Conducts monthly monitoring of assets and serves as point of contact for Able accounts; Helps to ensure people supported are not over resourced, and at risk of losing various benefits"

Managerial Duties:

1. Arranges and teach classes to staff on the Person Centered Planning, BSP and HRAC processes and quality expectations set forth by DDA and internally for documentation and writing operational goals.

2. Coach staff on writing and monitoring plans PCP, BSP and HRAC documentation.

3. Collaborates with Systems Implementation Specialist to ensure document storage is clear and efficient; Ensures PCP documents are stored electronically in TWINSIS (or the current electronic health record).

4. Supervises the PCP planning process and ensures written protocols are in place to support current procedures and practice.

5. Conducts trainings for all involved staff on the current regulations related to the Person Centered Planning Process.

6. May serve as a community liaison to all outside agencies and persons involved with our individuals.

7. Assures all staff communications are conducted in a professional and timely manner.

8. Elevate matters discovered involving violation of fundamental Human rights of people supported.

Supervision:

Provides indirect Supervision of the Person Centered Planning process for Program Managers, Team Leaders and other involved staff.


Qualifications:

A Bachelor’s Degree from an accredited College or University and one year of experience in providing services to adults with mental retardation or a high school diploma and at least three years relevant experience supporting people with I/DD in community settings. Must have demonstrated ability to develop and coordinate trainings and PCP meetings. Experience interacting with a variety of disciplines including medical professionals, clinical staff, family members, education institutions and other stakeholders. Must be computer literate and have excellent oral and written communication skills.


Mental, Physical, & Visual Demands:

In this position the employee should be able to lift a maximum of 20lbs and have knowledge of computers and calculators.


Working Conditions:

Approximately eighty percent of this position requires travel throughout Prince Georges County and the Washington Metropolitan Area. It may require driving through hazardous weather or heavy traffic. This position may require prolonged standing up from 4-8 hours depending on the employment worksite needs. This position may also require lifting from 10 to 50 pounds. Work may vary in weather conditions, temperatures, and various noise levels depending on the employment site.
