JOB DESCRIPTION: LICENSED VOCATIONAL NURSE (LVN) WELLNESS DIRECTOR
POSITION SUMMARY: Utilizes the nursing process to perform skilled nursing functions in accordance with the Nurse Practice Act, Federal and State guidelines. Provides supervision to staff of care to patients in the facility. Provides services in accordance with Glen Park’s philosophies, standards, and practices.

A. QUALIFICATIONS
1. Graduate of an accredited School of Nursing.
2. Legally-Currently licensed by the State Board of Nursing to practice as a Licensed Vocational Nurse.
3. Experienced as an LVN with one of the following:
a. 1 year of acute care experience.
4. Current CPR and First Aid Card.
5. Health/physical clearance within 6months; TB test clearance.
6. Criminal Background clearance.
7. Good written and verbal communication skills.
B. Duties
1. Responsible for planning, organizing, directing, evaluating, and overseeing the provision of resident care activities within the facility
2. Ensuring that subordinates provide the consistent delivery of timely and appropriate high-quality assistance, care, and medication management to all residents.
3. Directly responsible for developing and revising resident care plans, in accordance with established practices and policies of Glen Park and the requirements of Title 22 (DSS) & Title 17 (DDS).
4. Assist with med pass.
5. Ensuring appropriate documentation and reporting of resident condition.
6. The position further serves as a facility representative on resident care matters, interfacing directly with outside medical professionals, regulatory agencies, and suppliers.
7. Perform assessments on Admission and residents upon discharge from hospital settings periodically.
8. Responsible for achieving specific operating, compliance, program and client satisfaction results within cost and schedule constraints.
9. Assist the Administrator with various tasks relating to staff training in relations to care topics and medication administration topics and monitoring of care staff as needed.
10. Assist the Administrator with various tasks relating to training and monitoring of care staff as needed.
11. Attend daily stand-up meetings and monthly case management meetings to report care updates on all residents.
12. Monitor residents and make sure they DO NOT have prohibited health conditions. See policy.
13. Maintain Weight Record of all residents & report sudden weight changes to Administration.
14. Complete Weekly Progress notes for residents.
15. Monthly head-to-toe assessments for all residents.
16. Perform assessments on Admission and residents upon discharge from hospital settings periodically.
C. LINES OF SUPERVISION
1. SUPERVISION GIVEN & TO WHOM
a. Has authority to delegate responsibilities to the Direct Care Supervisor/Med Aide while adhering to Title 22.
2. SUPERVISION RECEIVED & TO WHOM

D. PHYSICAL REQUIREMENTS
1. Must be physically able to lift and pull up to 20 lbs., bend, stretch, and climb stairs. Support residents in turning, standing, sitting, transferring, and walking activities.
Job Type:Part Time
Shift Saturday and Sunday 6:00 AM-2:30 PM.
