Join our dedicated pharmacy teamin Wilkes-Barre, PA, where we focus on providing exceptional care to the community we serve. We are currently seeking a highly skilled Long Term Care Staff Pharmacist to support our mission of delivering comprehensive pharmaceutical services.

Job Description: As a Long Term Care Staff Pharmacist, you will play a crucial role in the management and delivery of medication therapy to long-term care residents. You will work collaboratively with healthcare providers to ensure optimal medication management tailored to the specific needs of elderly patients.

Key Responsibilities:

  • Review, prepare, and dispense medications for long-term care residents, ensuring accuracy and compliance with regulations.
  • Provide pharmacological information by answering questions and requests of health care professionals and counseling residents on drug therapies.
  • Develop pharmacological protocols and participate in medication therapy management.
  • Monitor patients medication therapies and provide interventions to prevent or resolve drug-related problems.
  • Collaborate with a multidisciplinary team to assess, plan, implement, and evaluate patient care plans.
  • Maintain records such as pharmacy files, patient profiles, charge system files, inventories, and registries of poisons, narcotics, and controlled drugs.
  • Ensure safe and secure storage of medications.
  • Conduct drug utilization reviews and prepare reports that detail findings and recommendations.
  • Educate staff and patients on drug administration and safety.
  • Stay updated with advancements in pharmacology and changes in drug regulations.

Shift:

  • Monday - Friday:11:00am - 7:00pm
  • Possible weekends.

Qualifications:

  • Pharmacy experience.
  • Pennsylvania Pharmacy license.
