The Community Health Center Network (CHCN), sister organization to the Alameda Health Consortium, is made up of the eight federally qualified community health centers in Alameda County. CHCN functions as the managed care MSO for our health centers, contracting with the Alameda Alliance for Health. In order to help our health centers meet their missions of providing the best possible care to their communities, we also provide a broad range of training, technical assistance, quality improvement, information technology, and data analytic services.

Position Title: Nurse Residency Program Manager

Department: Utilization Management (UM)

Reports To: UM Director

Classification: Exempt

Status: Full-Time (Temporary), Remote/Hybrid

POSITION SUMMARY

The Nurse Residency Program Manager supports both CHCN and AHC staff with the Nurse Residency Program. The Program Manager provides support to the UM Director and applicable Case Management (CM) and UM staff on operational issues and authorization functions and supports UM/CM training and operational needs as it relates to the Nurse Residency Program. This position also participates in outreach activities, agency advocacy, and serves on ad hoc committees, as requested. The Program Manager integrates the Nurse Residency Program into existing operations and assists with developing and overseeing curriculum that focuses on patient outcomes, leadership in the management of patient care, and the professional role of the nurse.

ESSENTIAL POSITION RESULTS

  • Plans, implements, and manages the Nurse Residency Program under the direction of the UM Director as it relates to CHCN.
  • Applies knowledge and experience to perform a wide range of activities and/or determines how to use resources to meet schedules and goals.
  • Coordinates formal/informal educational offerings for residents/program staff.
  • Maintains required information related to the Nurse Residency Program.
  • Functions as a liaison between CHCN, AHC and the Nurse Residency Program team in assuring a coordinated residency program.
  • Participates in conference calls, in-person and virtual meetings, seminars, advisory and work groups to create a professional network, share expertise and best practices, and assist with further development of the Nurse Residency Program.
  • Performs other job-related duties as assigned.
