Position Summary: We are seeking a dedicated Resident Advocate and Admission Coordinator to champion the rights and wellbeing of our residents at all times. This role plays a crucial part in evaluating and ensuring the appropriate level of care for new residents, maintaining communication with families and guardians, and managing the admission and discharge processes. The ideal candidate will thrive in a role where compassion, organization, and regulatory compliance are essential.

Essential Duties and Responsibilities:

  • Act as the primary liaison between Heinzerling Community, residents' families, protective services, and Service and Support Administrators.
  • Assess prospective residents' needs through comprehensive evaluations to determine suitability for facility placement.
  • Coordinate admissions meetings and facilitate visitations for prospective residents.
  • Ensure compliance with county and state admission regulations for smooth transitions.
  • Assist families with Medicaid, Medicare, Social Security, and insurance paperwork.
  • Obtain necessary consents from guardians upon admission and as required.
  • Prepare and maintain social histories for residents, participating in major unusual incident processes.
  • Collaborate with teams to review and update the Ohio Individual Service Plan and conduct Annual Options Counseling.
  • Manage residents' financial accounts for individual purchases and oversee inventory distribution.
  • Serve as the liaison with the Ohio Department of Disabilities for level of care evaluations.
  • Conduct training sessions on residents' rights and provide information during staff orientations.
  • Keep supervisors informed on census updates and efforts to fill vacancies.

Qualifications:

  • Bachelor's degree in Social Work or related field from an accredited college or university.
  • Minimum of two years of related experience and/or training, preferably with individuals with intellectual and developmental disabilities.
  • Ability to pass the DODD test for level of care evaluations.
  • Current licensure to practice social work in the state of Ohio preferred.

  • Proficiency in reading, interpreting, and applying regulations and procedures.
  • Critical thinking and problem-solving abilities.
  • Comfortable handling sensitive information with discretion and diplomacy.

Salary: Starting at $55,520 per year, commensurate with experience.
