POSITION TITLE: HOME CARE CERTIFIED NURSES AIDE

FLSA STATUS: NON-EXEMPT

REPORTS TO: Assistant Clinical Team Leader or Clinical Team Leader

POSITION OBJECTIVE:

To provide safe personal care and support for the terminally ill under the direction of the Patient Care Director and/or RN Case Manager.

QUALIFICATIONS:

  1. High school diploma or GED required.
  2. Completion of 105 hours CNA Certification program (need a copy of Certificate).
  3. Current/active CNA License AND
  4. Minimum six (6) months (one [1] year preferred) CNA/HHA experience within the last twenty-four (24) months in the home health, long-term care, or hospital setting. OR
  5. RN nursing students who have successfully completed courses with a grade of “C” or better and who currently possess an active CNA License will be said to have met the above qualifications.
  6. Must hold current CPR certification or obtain it within the orientation period.
  7. Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace.
  8. Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
  9. For CNA Home Care must be able to drive independently to all patient/family/ clients’ homes.
  10. Maintains a clean, safe, and healthy environment in the patient care area.
  11. As a member of the interdisciplinary team, participates in patient care conference as assigned to promote holistic care.
  12. CNA/Home Health Aide will maintain good personal hygiene and conduct themselves in a polite and professional manner.
  13. CNA/Home Health Aides must maintain a therapeutic relationship with patients. This includes confidentiality of personal matters as well as patients’, coworkers’, and Agency business.
  14. Maintains respect for the patient's privacy and property.
  15. Attends educational opportunities and in-services in accordance with agency guidelines to improve clinical practice.
  16. Must be willing to be cross-trained in other departments within the agency.
  17. Any other duties assigned by the supervisor.
  18. Adheres to all Stillwater Hospice policies.
  19. Adheres to Stillwater Hospice’s dress code policy.
  20. Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
  21. Participates in quality improvement activities and/or other agency committees or task forces when requested.

ESSENTIAL FUNCTIONS:

  1. Gives client care per agency policies and procedures, following a written Plan of Care.
  2. Appropriate and safe techniques in personal hygiene and grooming include bed bath, sponge bath, tub bath, or shower bath. Shampoo: sink, tub, or bed. Nail and skin care. Oral hygiene. Toileting and elimination needs, emptying commode and catheter bag; emptying, cleansing, and reinforcing ostomy bag. Normal range of motion and positioning, ambulating with the use of a gait belt. Changing linens, adequate nutrition, fluid intake, and output.
  3. Assists patients with various rehabilitation activities under the direction of a licensed therapist or RN.
  4. Takes and records temperature, pulse, respiration, and blood pressure when assigned.
  5. Meal preparations.
  6. Assists patients with feeding and feeding patients unable to feed themselves.
  7. Observe the patient’s condition and report significant changes in status to the appropriate nurse.
  8. Computer documents care per agency policies and procedures.
  9. Manages care of assigned patients in a timely fashion.
  10. Maintains a clean, safe, and healthy environment in the patient care area.

MARGINAL FUNCTIONS:

  1. Performs light housekeeping chores.
  2. May run errands, on occasion, with supervisor approval.

ESSENTIAL PHYSICAL/MENTAL FUNCTIONS:

  1. Must have visual, hearing and mental ability to initiate and comprehend written and verbal communication.
  2. Must be able to engage in clear, concise verbal/non-verbal communication.
  3. Lifting: Transfer patients into positions where they can perform daily care, personal hygiene, and grooming - Occasional to Frequent.
  4. Carrying: Personal hygiene supplies to and from bathroom to patient - Frequent.
  5. Linen for short distances - Frequent.
  6. Pushing/pulling: Readjust patient in bed - Frequent.
  7. Standing: While performing patient care - Frequent to Constant.
  8. Sitting: While performing patient care and paperwork. - Occasional
  9. Climbing: Up and downstairs in patient’s home - Occasional.
  10. Balancing: Needed for normal ambulation, climbing stairs, and reaching - Frequent.
  11. Stooping/kneeling/crouching/reaching: To perform patient care - Frequent.
  12. Handling patient care items, i.e. soap, shampoo, toothbrush, comb, dressing - Frequent.
  13. Fingering: Pen, visit reports - Frequent.
  14. Opening: Bottles and jars - Frequent
  15. Talking: To communicate with patient and family, and to communicate with supervisor and coworkers - Frequent.
  16. Hearing: To communicate with patients, coworkers, patients' families, and supervisors, Frequent to Constant.
  17. Seeing: Constant.
  18. Must be able to climb a minimum of three (3) flights of stairs.
  19. Recognizes emergencies and knowledge of emergency procedures according to agency policy.
  20. Must have the ability to remain calm in occasional emergency situations.

OTHER SPECIAL CONSIDERATIONS:

  1. Must be willing to accept assignments without regard to disease or ethnic origin.
  2. Must exhibit dependability in areas such as attendance, tardiness, and flexibility for unit coverage.
  3. Must be willing to work scheduled holiday rotation.

The published compensation range for this position reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. The base pay may be higher or lower based on demonstrated job-related experience.
