As the Community Intake Specialist, you will process referrals and meet with people supported and their families to ensure a seamless admission process. The Community Intake Specialist will be the initial contact of Momentum with people supported, families, regional center, and other outside vendors.

Community Intake Specialist Essential Responsibilities:

  • Promotes a friendly, comfortable, and safe environment
  • Respects and promotes the right of individuals
  • Assures continuity of person supported services with stakeholders and services provided by outside agencies
  • Serves as member of the Interdisciplinary (ID) Team and participates as a member of the Admission and Discharge committee
  • Attends all meetings regarding persons supported
  • Reviews all applications for admission to the facility and serves as a member of the Admission and Discharge committee
  • Seeks referrals for vacancies, circulates packets, coordinates pre-placement activities, maintains contact with referring agency, and seeks other placement for person supported as needed
  • Helps facilitate transfer process and provides potential person supported tours as necessary
  • Works closely with the Chief of Clinical Officer and nursing staff to ensure and coordinate smooth admission and discharge processes
  • Manages new clients for services waiting list
  • Reviews all bed holds regularly to update management on status and progress towards filling open beds
  • Works directly with regional center to ensure that potential persons supported are provided an opportunity for direct admission to a Momentum residential locations
  • Performs other duties as assigned to meet department and/or business needs

Community Intake Specialist Essential Qualifications:

  • Bachelor's degree in Human Services or Social Services, or equivalent years of experience; required
  • Minimum of one (1) year of experience working with persons with development disabilities, or related field; required
  • Experience with admissions or discharge; highly preferred
  • Experience working with Regional Center; preferred
  • Valid and current proof of legal right to drive in California; required
  • Strong organization skills including, time management and attention to detail
  • Strong communication skills including; reading and writing
  • Excellent customer service skills; highly preferred
  • Positive attitude with flexibility to meet the needs and demands of the organization
  • Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
  • Able to run, push, pull, bend, squat, kneel, and lift up to twenty-five (25) pounds
