JOB SUMMARY:

 Assess the needs of and provide medical care to patients. Adhere to Public Health Services guidelines and emerging policies and procedures regarding medical care for all individuals and the specialized population of persons living with HIV/AIDS and those requiring MOUD. This role requires working outside standard hours and the ability to operate at various locations.  

DUTIES AND RESPONSIBILITIES:

  • Assess and prescribe medication for patients with both acute and chronic conditions.
  • Assess, diagnosis, and treat patients of all ages.
  • Refer patients to urgent medical care, as needed.
  • Provide preventive services, treat illnesses and diseases that may occur.
  • Perform diagnostic tests and evaluate medical test results.
  • Coordinate with CMO and other staff to eliminate barriers to care for patient populations.
  • Manage overall patient care regarding lifestyle and development issues.
  • Stay up to date on community resources that are available to patients to promote holistic health and promote patient involvement in plan of care.
  • Effectively lead daily huddles for care coordination and continuity.
  • Effectively build and manage partnerships/relationships with outside sources to promote continuity of care.
  • Be flexible, organized and function under stressful situations.
  • Assure continuity and monitor patient care regarding Patient Centered Medical Home (PCMH) model.
  • Identify and address psychosocial needs of patients and family members.
  • Manage and perform all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Maintain a good working relationship and effective communication both within the department and with other departments for the benefit of the patient.
  • Participate in Continuous Quality Improvement (CQI) activities and daily huddles.

PROFESSIONAL REQUIREMENTS:

  • Adheres to dress code; appearance is neat and clean.
  • Complete annual education requirements.
  • Treat patients and their families with respect and dignity.
  • Communicate appropriately and clearly with all staff.
  • Maintain regulatory requirements, including all state, federal and National Committee for
  • Quality Assurance (NCQA) regulations.
  • Maintain and ensure patient confidentiality at all times.
  • Report to work on time and as scheduled.
  • Wear identification while on duty.
  • Maintain a good rapport and cooperative working relationship with all staff.
  • Represent organization in a positive and professional manner.
  • Comply with all organizational policies regarding ethical business practices.

SKILLS AND ABILITY:

  • Knowledgeable and experienced in medical management techniques, including safety; and
  • Well versed in clinical regulations and laws.
  • Able to motivate others, skilled in establishing and maintaining effective working relationships with other employees, patients, organizations, and the public. 
  • Able to function in a multi-professional environment.
  • Ability to cope under pressure and work to determined deadlines.
  • Expert knowledge and understanding of legislation relating to medical practice.
  • Highly developed knowledge and understanding of key organization systems and agenda.
  • Effectively communicate with physicians, patients, insurers, colleagues and staff.
  • Proficient in EMR, Microsoft Office, including Outlook, Word, and Excel.
  • Ability to work under minimum supervision and demonstrate strong initiative.
  • Ability to deal in an organized manner with problems involving multiple variables within the scope of the position.
  • Ability to communicate effectively in writing, over the telephone, and in person.

SUPERVISORY RESPONSIBILITY:

  • N/A

QUALIFICATIONS:

  • Successful track record delivering high quality services in a growing complex multi-disciplinary healthcare environment.
  • Must be able to perform each essential duty satisfactorily.
  • Knowledge of HIPAA compliance.
  • Understanding the ethics of confidentiality.
  • Must submit to annual screening/vaccination of Tuberculosis and Hepatitis B and any vaccinations recommended by OSHA Guidelines.

EDUCATION/EXPERIENCE:

  • Minimum of a Masters in Nursing
  • Minimum of 3 years’ experience

LANGUAGE SKILLS:

  • Able to communicate effectively in English, both verbally and in writing.
  • Additional languages preferred.
  • Knowledge of Electronic Medical Record system.

MATHEMATICAL SKILLS:

  • Ability to work with mathematical concepts such as probability and statistical inference.  Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY:

  • Ability to define problems, collect data, establish facts, and draws valid conclusions.  Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

CERTIFICATES/LICENSES/REGISTRATIONS:

  • Certification from the American Academy of Nurse Practitioners (AANP) or American Nursing Credentialing Center (ANCC).
  • Graduate of an accredited school of nursing with care management/ambulatory care experience preferred.
  • Current APRN and RN license within the state of practice.
  • Current BLS certification.
