Job DescriptionJob Description

Provides services and supports to individuals in all environments according to each person-centered plan. Recognizes natural learning opportunities and uses those to teach independent living skills, enhancing individuals’ independence. To maintain the individual’s home within established standards, including health and safety guidelines, protection of individual rights, and fiscal parameters. Will follow Rule 50, Dept. of Human Services Rule 115, Rule 116, Rule 119, Rule 120, and any other relevant regulatory guidelines.

Essential Duties/Responsibilities

  1. To support the philosophy of the agency mission, vision, values, and strategic plan.
  2. Manage all responsibilities based on the agency mission, using individual choice and all appropriate policies, procedures, and regulations.
  3. To conduct oneself in a manner that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.

Program/Department Development and Coordination

  1. Advocates for individuals through knowledge of personal references, protection from abuse and neglect or other rights violations, and supporting the development of personal relationships.
  2. Assist the individuals in striving toward independence in accordance with goals and objectives in the Personal Plan on the individualized Service Plan
  3. As a mandated reported, ensure allegations of abuse, neglect, and exploitation are reported as required by EPC policy and government regulation
  4. When necessary, assist individuals with personal needs such as transferring, toileting, eating and grooming
  5. Delivers services and supports as outlined in everyone’s Person-Centered Plan ensuring that activities and supports reflect individual preferences.
  6. Assists individuals in maintaining their home, reporting needed repairs or modifications to the appropriate parties within established timeframes.
  7. Recognizes individual differences and adapts teaching/ interacting style to best support the person's needs.
  8. Assure that the basic civil, human and legal rights of all persons are respected
  9. Supervise the individuals in community and at home throughout assigned shift
  10. Assure dignity, privacy and rights of program participants are always observed
  11. Act as a positive role model for individuals at all times
  12. Provide natural learning opportunities to teach independent living skills in accordance with the principles of normalization
  13. Involve the person supported with a variety of in-home and community leisure activities in accordance with the principles of age-appropriateness and normalization
  14. Complete annual training refresher based on EPC policy and state regulations
  15. Attend all in-service meetings, House meetings and Discovery meetings designated by supervisor.
  16. As applicable, obtain and maintain “authorized status” as designated by the RN by successfully competing medication training and by competently and accurately administering and supervising medications
  17. Perform all safety and emergency procedures in a calm, accurate and professional manner
  18. Communicates regularly with the Community Living Manager, QIDP, nursing, and other team members regarding progress towards outcomes, health and safety, and other issues that may arise.
  19. Follow and implement all pertinent agency policies, departmental protocols, and procedures.
  20. Provide required documentation regarding program delivery, medication administration, financial records, and all other requested documentation.
  21. Provide needed transportation to people supported using an EPC vehicle or their vehicle as required.
  22. Maintain documentation on all client structure programs and goals on specified data collection forms at the frequency indicated on the ISP in accordance with pertinent CILA and agency guidelines
  23. Provide required documentation regarding program delivery, medication administration, financial records, and all other requested documentation
  24. Maintain progress note documentation in the areas of basic living skills, leisure activities and general progress as required
  25. Maintain accurate financial records by retaining all receipts of bank transactions and expenditures
  26. Maintain other necessary documentation (e.g. mileage sheets, GERs, etc)
  27. Practice safety procedures and maintain acceptable standards of personal hygiene.
  28. Report any concerns of the home environment (inside and outside) to assure the home is safe, clean and in orderly fashion.
  29. Request information, instructions, and guidance from Program Manager in order that effective program service delivery may be maintained.
