POSITION SUMMARY

The Home Visitor will serve as a team member of GCLC’s Home Visiting Program to provide families enrolled in home-based programming to provide strengths-based support, resources, advocacy, and crisis intervention in accordance with federal, state and program guidelines. The Home Visitor is responsible for providing child development, curriculum, and family support on home visits to families of infants and toddlers and expectant mothers. The Home Visitor collaborates with staff and community partners to provide comprehensive services, assists in the planning and implementation of family events/parent meetings, and assists in the recruitment and registration process. The Home Visitor maintains appropriate records and data in accordance with the Home Visiting Program and GCLC’s Performance Standards and established guidelines.

The Home Visiting “team” is by definition multidisciplinary and composed of licensed/credentialed professionals who have knowledge in early childhood development and early childhood mental health in combination with non-degreed professionals with appropriate backgrounds and expertise.

DUTIES AND RESPONSIBILITIES

  • Provide comprehensive case management to 12-20 home-based families.
  • Conduct weekly or bi-weekly home visits with enrolled families (frequency and intensity depend on the programming option the family is enrolled in).
  • Plan and implement home-based learning experiences that ensure effective implementation of the home visiting curriculum and promote child development.
  • Use assessment to identify areas of support needed and maintain and regularly update the plan for each enrolled family.
  • Complete child development screenings and assessments as required by program standards.
  • Collaborate with community organizations to provide support and resources for enrolled families to meet their goals.
  • Attend community outreach and recruitment events on behalf of GCLC’s Home Visiting Program.
  • Ensure case files and documentation are completed and maintained in accordance with Home Visiting Program and GCLC’s Performance Standards and established guidelines.
  • Actively participate in monthly state-wide reflective case consultation.
  • Actively participates in supervision meetings and Home Visiting Program staff meetings.
  • Assists in the planning and implementation of family events and parent meetings.
  • Complete ongoing professional development, management, life skills, and parent education.
  • Remain current with community resources and family friendly activities.
  • Approach their work from a family strengths perspective rather than a deficit perspective.
  • Display acceptance and respect for each family’s diversity and a cooperative spirit with coworkers.
  • Use personal transportation to provide services in a timely manner.
  • Maintain confidentiality of all information regarding clients, staff, physicians, and Guidance Center of Lea County (GCLC) as a result of employment at GCLC.
  • All other work related or duties assigned by the Home Visiting Supervisor, Program Manager or Director of Community Services.

QUALIFICATIONS

Minimum

  • High School Diploma/GED; or High School equivalency with a desire to pursue Higher Education in a related field pursuant to Home Visiting standards by both State & Federal recommended credentials.
  • Have knowledge of child development and understand and appreciation of the families’ social, economic and cultural background.
  • Have knowledge of the specific areas of focus of the program such as health, parent education, or special education services.
  • Have knowledge in pregnancy and the prenatal period, infant/toddler safety and health, early childhood development, along with early childhood mental health principles and practices.
  • Prior experience in the field of early childhood education; maternal/child health, child development and/or public health.
  • Prior experience in pre-natal, infant, toddler mental health and cognitive development or related area pertinent to expectant mothers and children up to age 5.
  • Self-starter with the ability to work independently and follow-up on all work assignments given.
  • Must have the ability to work in a flexible environment while meeting deadlines and program standards.
  • Must have strong communication, interpersonal, and relationship-building skills.
  • Exemplary organizational skills.
  • Must be computer literate with working knowledge of Word and Excel.
  • Maintain a valid and unrestricted NM Driver’s License.
  • Must be able to use personal vehicle over course of employment.
  • Maintain valid and sufficient personal automobile insurance.
  • Must pass required background checks and drug screenings.

Preferred

  • Fluency in English and Spanish, both verbal and written.
