Join Our Team Re-entry Case Worker Needed

We are seeking a dedicated Re-entry Case Worker.

Position Summary:

As a Re-entry Case Worker (Grant Funded), you'll provide essential post-release support services, helping clients navigate the transition from incarceration back into the community. This role is a unique blend of community health work and case management, where you'll conduct outreach, provide health education, and serve as a vital link between individuals, the health center, and social service providers. This grant funded position will be an employee of both the CHCFC and the Franklin County Sheriff's Office.

Key Responsibilities:

  • Offer case management and support to clients from the Franklin County Jail and House of Correction.
  • Identify client needs, set goals, and implement tailored interventions.
  • Monitor client progress and adapt support as they reintegrate into the community.
    • Facilitate behavioral health groups focused on reentry.
    • Conduct intake interviews and manage data for reporting purposes.
    • Collaborate with interdisciplinary teams to improve patient health outcomes.
    • Establish strong relationships with community partners and agencies.

Requirements:

  • Bachelor's degree or related level of experience equivalent to four years of college
  • Commitment to CHCFC's mission and adherence to our partner organization's policies.
  • Obtain CITI Cert per IRB requirements with 30 days for providing informed consent for research purposes.
  • Certification as MassHealth Navigator required within 90 Days.
  • Experience working in case management.
  • Valid driver's license and ability to operate FCSO vehicles.
