Full Time Days Working 8am-4:30pm

The Social Worker Designee supports our mission and values by organizing, planning, and implementing the social service programs for the residents at Brethren Care. Identify and understand the social and emotional factors underlying residents' illnesses and communicate these factors to the health care team. Assist residents and their families to understand and accept treatment necessary to provide maximum medical benefits. Assist residents and their families to adjust to permanent and temporary effects of illness. Provide direct social service to residents and their families.

The Social Worker Designee's primary responsibilities will include:

  • Meet resident and family on admission and complete a Social History and Assessment.
  • Participate in RCC meeting and report social service needs of residents and methods of meeting those needs.
  • Maintain a written social service plan for each resident. (MDS, CAAs, Care Plans).
  • Update social service progress notes as needed (at least every three months).
  • Invite competent residents to RCC and if they decline, inform them of the outcome at their request. At family/POA request, if they are unable to attend, discuss outcome of RCC meeting.
  • Assist in making arrangements for transportation to hospitals, clinics, doctor's offices, etc., when such is required.
  • Provide individual, small group, and family counseling services.
  • Help resident/family with billing, financial/MCD questions, problem solving and referral.
  • Participate with nursing department in assisting families to purchase special equipment such as wheelchairs.
  • Participate with nursing department to make room assignments.
  • Participate with nursing department in discharge planning, i.e., making referrals to appropriate community resources.
  • Assure confidentiality of all residents' records and the sharing of information with other care agents or agencies, obtain expressed written consent before any disclosure is made to any person who is not a participant in the Brethren Care network.
  • Act as an advocate for any resident who is without a suitable family member or friend who could assist in assuring resident's rights. Assist residents to secure such an advocate whenever possible.
  • Attend Quality Assurance and submit written report when asked.
  • Review Resident Rights with resident council if not arranged with Life Enrichment or Ombudsman.
  • Substitute Chair of Grievance Committee, and meeting one time a year.
  • Assist in locating missing items. Fill out report.
  • Obtain services for residents to acquire dental care, glasses/contacts, and hearing aids.
  • Assist in resolving family conflict.
  • Assist residents in obtaining POA, living will. Explain Advanced Directives as necessary.
  • Help residents obtain physician if necessary.
  • Communication Board for non-verbal residents.
  • Assist residents in keeping in contact with families/friends by phone or written correspondence.
  • Assist residents and families in obtaining invalid clothing.
  • Obtain therapeutic and adaptive information to enable residents to function at the highest level.

What is required in this position?

  • Experience in social work preferred.
  • Understands and applies techniques/skills for therapeutic intervention and improved social functioning of residents.
  • Positive attitude and compassion toward aging persons.
