POSITION OVERVIEW
A Care Manager provides direct care to individuals served (ages 5-17 years old) with behavioral health needs that include the process of triage, assessment, planning, and communication. Participates as a member of a multidisciplinary treatment team in the Children's Emergency Crisis Stabilization facility.

JOB REQUIREMENTS:
- Must have a bachelor's degree in a related field of study.
- Must be able to lift and carry up to 50lbs of dead weight
- Must have excellent interpersonal skills
POSITION RESPONSIBILITIES
- Conduct thorough assessments and arrange community resources based on patient needs following program requirements.
- Ensure timely completion of all clinical and administrative documentation as per program guidelines.
- Communicate effectively and foster positive relationships with internal and external resources.
- Create service plans, treatment plans, and aftercare plans with clearly defined goals and objectives as per program requirements.
- Manage workflow efficiently and maintain organization according to program guidelines.
- Uphold high-quality clinical documentation as per program requirements.
- Perform any other duties as assigned or needed as per program requirements.
WORK ENVIRONMENT:
- Secure facility that provides inpatient treatment programs for those experiencing a mental health emergency needing 24-hour care in a safe supportive environment.
- This is a 28-bed Children's Crisis inpatient served care facility.
- Maintains confidentiality and remains calm under stressful situations.
