JOB DESCRIPTION
The Patient Monitoring Associate creates a memorable onboarding experience for our patients enrolling in the RPM Home Monitoring Program. The Patient Monitoring Associate educates all patients about the program, how it will positively impact their quality of life, and how to use our medical devices, such as a blood pressure machine, glucose monitor, and weight scale.
- Assist patients during the on-boarding process, training, and monitoring phases of the Remote Patient Monitoring program.
- Enroll patients in the Remote Patient Monitoring program.
- Educate each patient on the usage of assigned equipment.
- Monitor patients vital signs and ensure daily uploads of readings of their assigned equipment.
- Communicate with patients via text, phone and video calls.
- Monitor patients vitals to meet provider's protocols and long-term health goals.
- Follow up with patients to ensure all protocols are being followed and patient participation in the program.
- Create a relationship with patients assigned, ensuring their participation and commitment in the program.
- Complete any additional tasks your manager may assign.
WORK EXPERIENCE
- Two-Year minimum of clinical experience is desired. Internship equivalent accepted
- Experience educating patients regarding chronic conditions
QUALIFICATIONS
- Ability to work independently while maintaining accuracy and consistency with processes and patients assigned
- Ability to be discreet and maintain confidential information
- Ability to communicate effectively and interact at various organizational levels and with a diverse range of individuals to include patients and providers
- Ability to pass drug screenings and background checks.

Compensation:
