We place a strong emphasis on individualized care, assisting residents with daily tasks while empowering their independence.
What does a Care Manager do?As a Care Manager at The Rivers At Maryland Farms, you will be responsible for providing personalized assistance to residents with their daily activities, including medication management, meal assistance, and personal care. Collaborating with a multidisciplinary team, you will create individualized care plans, ensure resident safety, and maintain accurate documentation. Additionally, you will engage with residents to promote socialization and well-being, as well as respond to any emergent needs or concerns promptly and effectively.
Your role will require compassion, excellent communication skills, and the ability to adapt to evolving resident needs. You will play a crucial role in enhancing the quality of life for our residents and fostering a supportive and caring environment.
What you need to be successfulTo excel as a Care Manager at The Rivers At Maryland Farms, you must have a genuine desire and affinity for working with the elderly, demonstrating compassion and empathy in all interactions. You should be comfortable maintaining a multi-disciplinary team approach to healthcare and have the ability to cultivate strong working relationships with staff, residents, and families. Effective planning, organizing, directing, and managing skills are key to successfully carrying out daily tasks and ensuring the well-being of residents. Knowledge of current regulatory standards and a high school diploma or GED are essential qualifications. A minimum of six months of caregiving experience, particularly with the elderly, is preferred to thrive in this role and provide top-quality care to our residents.
