REQUIREMENTS for a Behavioral Health Clinician with Lutheran Social Services:

  • I have an MSW (Masters degree in Social Work)
  • I am a LISW in good standing
  • I have 2 or more years experience working in a social service setting
  • I have 2 or more years of experience working in a health care setting
  • I have experience utilizing (minimally) the following techniques, interventions and EBP's: Motivational Interviewing skills, Stages of Change, Harm Reduction.
  • I have experience conducting diagnostic assessments
  • I have experience working with following or related populations: homelessness, behavioral health, substance abuse/addiction, chronic physical disabilities, HIV/HIV prevention & education strongly preferred
  • I have administrative reporting experience

RESPONSIBILITIES for a Behavioral Health Clinician with Lutheran Social Services:

  • Participates in the support and implementation of effective and quality Behavioral Health and Case Management services to be provided to the patients of The Center at Faith Mission.
  • Coordinate patient services with medical staff in The Center at Faith Mission to ensure the delivery of patient focused, integrated health care.
  • Conduct comprehensive diagnostic assessments with assigned and appropriate clients entering Faith Mission's emergency shelters as needed.
  • Conduct screening interviews for patients with The Center at Faith Mission and coordinate care plans with the medical staff.
    • Determine behavior health needs including AOD level of care.
    • Coordinate and link clients with appropriate Case Manager and intervention groups.
    • Develop initial treatment plan based on diagnosis, strengths and needs.
    • Provide short term intervention to improve health outcomes
  • Complete pre-screens, intake paperwork and diagnostic assessments specific to the enrollment of individuals in the medication assisted treatment program.
  • Provide ongoing clinical services, such as counseling, group facilitation, case management to support individuals enrolled the medication assisted treatment program.
  • Participate in weekly team huddles to conduct case review of clients enrolled in the medication assisted treatment program.
  • Maintain complete and accurate documentation related to the assessment including client questionnaires, Faith Mission Diagnostic Assessment, Lethality Assessment, Mental Status Exam, Admission Verification for Outpatient Level of Care, Initial Treatment Plan and others as deemed necessary. Maintain clear and precise data and progress notes, including timely submission of CQI reports and careful documentation of all client interactions. Ensure timely collection and collation of statistical information. Ensure that all documentation meets HRSA and CARF Accreditation Standards for clinical competency.
  • Assist with crisis intervention and de-escalation by providing leadership and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.
  • Actively participate in continuous quality improvement meetings, working with colleagues toward positive client outcomes.
  • In conjunction with the Director of Clinical and Supportive Services and The Center Director provide supervision to student interns to enhance their educational experience by providing quality field instruction while also benefiting Faith Mission programs and fulfill learning agreements with institutions of higher learning.
  • Provide specialized training to all Faith Mission staff. Provide other clinically oriented trainings to Faith Mission staff and others as needed and required.
  • Maintain positive relationship with residents, shelter staff, volunteers and visitors.
  • Maintain existing and grow new and existing relationships with collaborative community partners that provide onsite specialized supportive services (Legal Aid, Veterans Administration etc.).
  • Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
  • Perform all other duties as assigned.
