Job Description

POSITION TITLE: Qualified Intellectual Disabilities Professional (QIDP)

DIVISION: Community Living 

SUPERVISOR: Assistant Director, Community Living

CLASSIFICATION: Exempt

PURPOSE:

Integrate, coordinate, and monitor each client’s active treatment program. Provide supervision to the Team Managers to achieve the goal of ensuring that the client receives necessary services and interventions.  Ensure that the individual receives those services and interventions necessary by competent persons capable of delivering them.

KEY AREAS OF RESPONSIBILITY:

  1. Provide pre-admission services necessary for client’s entrance into group living services, including orientation process.
  2. Serve as case manager for residential clients by developing and monitoring IPP.
  3. Coordinate the delivery of the approved IPP services.
  4. Provide individual specific training to new residential staff and provide updated trainings as persons served needs/requirements change.
  5. Ensure that individual specific training information is current in all client’s files.
  6. Attend and participate in monthly house meetings for clients.
  7. Ensure that all required annual paperwork for clients is completed. Ensure that program goals reflect the clients ISP.  
  8. Review monthly data documentation and write monthly progress reports. Ensure that the monthly progress reports for all residential services are completed in a timely manner.   
  9. Develop, monitor implementation, and follow-up on each clients Individual Support Plan (ISP)/Individualized Program Plan.
  10. Schedule and chair interdisciplinary staff meetings, as needed for program planning and review to meet program standards.
  11. Complete file maintenance of all central and on-site files. Complete file closing in accordance with regulations.  
  12. Coordinate and evaluate the instructional activities that are provided to clients.
  13. Display patience in all situations involving clients. Speak in an even, positive, and personal tone of voice when conversing with clients.  
  14. Appropriately respond to client’s behavior via verbal and/or physical intervention based on the needs of the client.
  15. Provide services in compliance with all funding sources and governing bodies (CARF, BQIS, State Board of Health, State and Federal Law).
  16. Ensure that employees follow all departmental and agency policies as well as state and federal laws.
  17. Maintain the confidentiality of clients.
  18. Participate directly in the recreational, therapeutic, and training activities of clients.
  19. Serve as liaison between clients, families, staff, professionals, and the community.
  20. Safeguard clients and families’ human and civil rights.
  21. Orient and train staff in all programs, using lecture, role model, text, and outside sources.
  22. Complete BDDS incident reports and needed follow-ups.
  23. Apply current BDDS standards.
  24. Assist person served with finances.
  25. Assist in the scheduling of staff to work appropriate shifts if necessary.
  26. Must be capable of meeting the physical demands required by caring for the persons served. Physical requirements may include, but not be limited to bending; reaching; stretching; lifting 25-50 pounds (depending on person served) without assistance; grasping; visual acuity; running; walking; squatting, guiding; transporting; assisting with physical transfer; and manual dexterity to handle medications; and handling physical aggression as defined in CPI training.  
  27. Supervise Team Managers as requested per Regional Director.
  28. Audit and approve the time of Team Managers. Assist Team Managers in auditing and approving DSP time, as needed. Audit and approve mileage for all Supported Living staff.
  29. Promote agency philosophies, generate team spirit, create, and maintain an environment that fosters employee satisfaction, maximizes productivity and profitability.
  30. Plan and provide instruction in communication skills, daily living, socialization skills, fine and gross motor skills, perceptual skills, prevocational skills, and any other areas based on programming needs as needed.
  31. Implement programs in a timely fashion.
  32. Route any receipts to the Administrative Assistant.
  33. Provide direct care when needed.
  34. Support Residential Advisors with supervisory oversight.
  35. Document personnel issues and route to Director, Community Living.
  36. Interview prospective direct care staff as needed.
  37. Research and submit accurate billing as needed.
  38. Provide clear feedback to interested parties.
  39. Become active in an organization of related professionals.
  40. Attend related training as authorized.

ESSENTIAL JOB SKILLS:

  1. Excellent and professional communication skills, both oral and written.
  2. Ability to perform budgeting calculation and comparisons.
  3. Must possess critical thinking skills.
  4. Complete other duties and responsibilities as assigned.
  5. Comply with ethical standards of DSI, FSSA, and other governing bodies.
  6. Ability to work flexible, non-standard hours, including occasional evenings and weekends.
  7. Must possess excellent telephone skills.
  8. Ability to effectively operate equipment such as a scanner and copier and have a working knowledge of computers and related software (i.e. Microsoft Office)

REQUIREMENTS:

  1. Bachelor’s degree in human service field required
  2. At least one year of experience working with the DD population required, preferably in a residential setting.
  3. Must possess or be able to obtain CPR, NCI, and first aid certification annually and perform as necessary.
  4. Must have excellent oral and written communication skills and maintain confidentiality with sensitive information.
  5. Maintain a valid Indiana driver’s license, insurance on personal vehicle (minimum of PLPD with medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.
