In this role, you'll focus on enrolling individuals with diabetes or hypertension into our Chronic Care Management (CCM) program, a benefit provided by their insurance company. As the first point of contact, you'll manage a high volume of inbound calls, ensuring each caller receives a positive and supportive experience.

Key Responsibilities:

  • Maintain accurate documentation of all communications in patient records.
  • Coordinate and schedule follow-up physician appointments through an EMR (Electronic Medical Records) system.
  • Liaise with physicians and other healthcare providers.
  • Provide efficient and accurate responses to callers, offering solutions and referrals while ensuring clients feel valued.
  • Utilize software, databases, scripts, and tools effectively.
  • Participate in training and learning opportunities to enhance your knowledge of the company and your role.
  • Adhere to all company policies and procedures.

Skills Required:

  • Experience in a healthcare call center or medical practice environment is required.
  • Proficiency in using Electronic Health Records (EHR) systems
  • Strong time management skills and ability to multitask and prioritize effectively.
  • Social perceptiveness and service-oriented mindset.
  • Bilingual proficiency in English and Spanish.
  • Certified Medical Assistant
  • Proficiency in MS Office suite.
  • Exceptional customer service, active listening, and professional phone demeanor.
  • Experience in Endocrinology or chronic care management preferred
