We have a wonderful Physical Therapist opportunity at one of our partner Skilled Nursing Facilities in Walnut Creek, CA. This position is in-person and is looking for a ASAP start for a 13 week contract.

You will play a crucial role in helping patients regain physical mobility, improve functional abilities, and enhance their overall quality of life. Your expertise will be vital in developing and implementing personalized treatment plans, assessing patient progress, and collaborating with a multidisciplinary team to provide comprehensive care.

Key Responsibilities:

  • Provide direct physical therapy services to patients in medical settings such as skilled nursing facilities (SNFs), home health, outpatient clinics, or acute rehabilitation centers.
  • Conduct comprehensive evaluations to assess patients' mobility, strength, balance, and functional abilities.
  • Develop and implement individualized treatment plans to address patients' specific needs and rehabilitation goals.
  • Administer therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function and mobility.
  • Collaborate with interdisciplinary healthcare teams to coordinate patient care and optimize outcomes.
  • Educate patients and caregivers on therapeutic exercises, home exercise programs, and strategies for maximizing functional independence.
  • Monitor patients' progress throughout the course of therapy, adjusting treatment interventions as necessary.
  • Document patient evaluations, treatment sessions, and progress notes accurately and in compliance with regulatory standards.
  • Participate in discharge planning and transition of care processes to facilitate patients' safe and successful return to their communities.

Requirements:

  • Doctoral or Master's degree in Physical Therapy from an accredited program.
  • State licensure or certification as a Physical Therapist.
  • Experience or specialized training in medical settings such as skilled nursing facilities (SNFs), home health, outpatient clinics, or acute rehabilitation centers preferred.
  • Knowledge of physical therapy interventions, techniques, and modalities used in medical rehabilitation.
  • Ability to collaborate effectively with interdisciplinary healthcare teams to coordinate patient care.
