Clinical Coordinator /Program Director of the Barbara Van Buren Housing Program

The Director of BVB reports to the Director of Clinical and Social Services.

Program Director Responsibilities:

  • Oversight of the Barbara Van Buren Housing Program, an 18-bed program that provides supportive housing to individuals with a diagnosis of HIV. Ensure compliance with the requirements of the HIV/AIDS Service Administration (HASA) and the NYC Department of Homeless Services (DHS).
  • Conduct/oversee the intake process and ensure that the program meets the contract’s required occupancy rate.
  • Provide clinical and case management supervision to the two Barbara Van Buren Housing Program staff (Social Worker and Case Manager).
  • Supervise the medication management program.
  • Administration of personnel policies and procedures for the program, and office management.
  • Participation in the formation and implementation of WSFSSH policies at Euclid Hall.
  • Collaboration with the Euclid Hall Building Manager, Euclid Hall Program Director, and Euclid Intake Coordinator.
  • Participation in meetings and programs with the WSFSSH Upper Management and/or Leadership Team
  • Other responsibilities upon mutual agreement with the Program Director of Euclid Hall and the Director of Clinical and Social Services.

Clinical Coordinator Responsibilities:

  • Provide clinical and case management supervision to social work and case management staff.
  • Provide access to clinical supervision to all staff as needed.
  • Provide clinical support and resources for any psychiatric emergencies.
  • Review and sign support plans, case management summaries, case notes, and psychosocial histories.
  • Ensure compliance with contract and regulatory requirements through regular chart audits to review completeness, timeliness, and quality of documentation.
  • Meet regularly with the Building Manager to review tenant and building issues.
  • Coordinate with the Building Manager on staff training and provide resident-related counseling to building staff as needed.
  • Lead a weekly/bi-weekly team meeting with social work staff.
  • Facilitate monthly or quarterly meetings with the Building Manager and social work staff.
  • Provide orientation and training for new social work staff.
  • Review incidents and incident reports and submit to contracted agencies as required.
  • Assume leadership role during site audits by contract agencies; write and submit any required Plans of Correction.
  • Supervise social work intern/s as needed.
  • Develop a 1:1 relationship with residents
  • Assist residents in obtaining and maintaining appropriate medical and mental health services.
  • Assist residents in obtaining and maintaining entitlements.
  • Assist residents in establishing and/or maintaining family and social contacts.
  • Assist residents in identifying socialization activities.
  • Complete required forms and reports for resident discharges and/or transfers.
  • Provide crisis intervention services.
  • Visit residents in hospitals, nursing homes or other care facilities as needed.
  • Facilitate community building through various modalities including group work services, activities, and holiday celebrations.
  • Other responsibilities as assigned.

Qualifications:

  • MSW required; LCSW preferred.
  • Knowledge about issues related specifically to HIV/AIDS such as (but not limited to) anti-viral medication options, laboratory testing (viral load and T-4 cells), transmission, etc.
  • Minimum of 3 years of supervisory and/or clinical experience in relevant settings
  • Able to work within a multicultural staff and resident environment
  • Flexibility and desire to work with a special needs population.
