Job Summary: Under limited supervision, provides a broad range of social services to consumers and their families. Conducts consumer intakes, participates in treatment team planning and performs crisis intervention. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May monitor operations of a community-based treatment, training or personal support residence. Functions as a supervisor or lead worker for other para-professionals.

Consumer Care

  • It is the responsibility of all staff to insure that the persons served through our programs receives services that address their individual needs.
  • Meets applicable productivity requirements in terms of direct service time: (Manager: Quote the actual productivity required).
  • Instructs and advises clients on daily living and work skills in one-on-one or group instruction.
  • Observes and monitors client behavior.
  • Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems.
  • Assists with the development of an individual service/treatment plan for each assigned consumer.
  • Participates in case reviews, consulting with the team to evaluate the client's needs and plan for continuing services. Counsels and advises clients on behavioral problems, daily decision-making, and the resolution of minor problems.

Employment Training

  • Responsible for prevocational training of consumers in a production setting.
  • Responsible for training and supervision of consumers in CAG activities.
  • Ensure consumers are trained according to ISP goals 100%.
  • Ensure recycling totals are figured accurately.
  • Ensure consumer time and pay is accurately recorded

Transporting

  • Operates vehicle to transport clients on routes, to program and home each day.
  • Transports clients to community access activities
  • Provide transportation to/from center and outings.
  • Ensure all passengers follow rules according to DMV.
  • Follow all laws and guidelines while driving and transporting consumers.
  • Ensure weekly inspections are completed on assigned vehicle. Ensure assigned vehicle is cleaned weekly.

Record Management

  • Prepares and maintains records of client progress and services performed, reporting changes in client condition to manager or supervisor.
  • Ensure all documentation is accurate according to consumer goals and objectives.
  • Ensure all reviews and annual paperwork is completed prior to review date.
  • Required to document services within 24 hours after service is provided. Upon request Instructor should send documentation to supervisor in the form for and alert for them to review and release to billing. All documentation of services will address ISP goals and will correspond with amount and frequency outlined in ISP. All documentation will clearly reflect skill building that is appropriate for the service being billed.

Required Education & Experience: Five years of full-time (or equivalent part-time) experience in social services delivery (case management, outreach, prevention/education, etc.) or Completion of 90 quarter hours (or its semester equivalent of college coursework, 15 quarter hours (or its semester equivalent ) of which was in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field and four years of full-time (or equivalent part-time) experience in social services delivery or Completion of a bachelor’s degree in one of the coursework areas listed above and one year of full-time (or equivalent par-time) experience in social services delivery or Completion of a master’s degree in one of the coursework areas listed above or Two years of full-time (or equivalent part-time) experience as a Senior Social Services Technician.

Work Environment: This job operates in a variable business settings with trips into the community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This role provides basic employment support which requires employee to perform in loud/quiet environments, outdoors/indoors, etc. Some medium travel between Gateway sites and in the community is required.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Employee is frequently required to walk, sit, stand or kneel and occasionally required to climb or balance and stoop. Employee must frequently lift and/or move up to 15 pounds. Must have the ability to sit for long periods of time at a computer. Employee frequently uses fine hand/eye coordination, hearing and visual acuity. Lighting and temperature are adequate, and there are not hazardous or unpleasant condition caused by noise, dust, etc. Employee must be able to travel between Gateway sites.
