SUMMARY OF DUTIES:

The Community Support Professional provides in-home and/or community-based supports to individuals with developmental disabilities through CH. The Community Support Professional provides one-to-one or small group supports assisting individuals with relationship building, social skills, leisure activities and daily living skills to foster independence at home and in the community.

REPORTING RELATIONSHIP:

Reports directly to the manager or designee of the assigned service. This position has no supervisory responsibilities.

ESSENTIAL DUTIES:

1. Responsible for providing supports and services at the frequency outlined in the person’s staff action plan or plan of care.

2. Responsible for service recipients’ safety in the home and community, as outlined in the person’s identified safeguards.

3. Coordinates scheduling of services with individuals and families, in conjunction with program management.

4. Communicate effectively with individuals, families, and community relationships.

5. Responsible for transporting, accompanying, and assisting individuals in the community.

6. Identifies unmet needs and provides appropriate assistance to meet those needs with the person.

7. Attends all scheduled meetings and trainings.

8. Accurately completes required reporting, recording, and service documentation on a daily basis.

9. Participates in all required trainings including any required trainings by the managed care organization (MCO) and NYSDOH. Applies practices per training and demonstrates knowledge of service standards, rules and regulations.

8. Recognizes and immediately reports any safety concerns, including fire safety, physical plant, vehicles, safety of individuals, staff or others.

10. Models expected behavior in the community, at the worksite, and at recreational activities. Maintains professionalism at all times. Adheres to the DSP Code of Conduct.

11. Responsible for ensuring that all confidential and potentially sensitive information is processed, retained and utilized in a strict and confidential manner.

OTHER DUTIES:

12. Adheres to all agency policies and procedures.

13. Responsible for displaying sound, ethical judgment and supportive decision making as outlined in the DSP Code of Ethics.

14. Other duties as assigned.

QUALIFICATIONS:

To perform this job, successfully, applicant must be at least 18 years old, hold a valid New York State Driver's License with driving record that is acceptable within sasi’s criteria, and be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. NOTE: In addition to meeting OPWDD’s background check, this position requires the candidate to meet OMH and Office of Children and Family Services background checks.

EXTRAORDINARY WORK CONDITIONS:

Travel to the homes of participants, meeting locations, training, etc., is required and is a normal and expected aspect of the job. Use of personal vehicle may be required.

EDUCATION & EXPERIENCE:

High School Diploma or GED preferred. Experience working with children or adults with developmental disabilities preferred.
