Position Title: Medical Case Manager Specialist for LGBT Older Adults

Reports to: Program Coordinator

Department: Direct Services

Program: Direct Services

Status: Full time, Nonexempt, eligible for benefits

Qualifications:
  • Bachelor’s Degree (Master’s preferred) in Social Work, Psychology, Nursing or Human Services and one year of experience in providing Case Management Services.
  • Expertise in case management.
  • Experience working with adults over the age of 60.
  • Expertise in advocacy.
  • Ability to cultivate relationships with outside agencies/hospitals.
  • Ability to work with diverse populations.
  • Ability to manage crisis situations.
  • Ability to establish and maintain collaborative relationships.
  • Knowledge of local community resources and services.
  • Understanding of medical and psychosocial issues related to the unique combination of aging and HIV disease.
  • Flexibility in work schedule, including evenings and weekends.

Basic Function:

  • Provide comprehensive linkage to practical and emotional support services for clients by the establishment and development of a productive working relationship, based in trust and mutual respect.

Specific Responsibilities:

  • Perform intake and face to face case management assessments for the older adult population.
  • Provide information and referral services as required.
  • Complete comprehensive Service Care Plan in collaboration with client.
  • Obtain informed consent and relevant release of information from client.
  • Obtain medical, demographic and academic data on client.
  • Provide case management and advocacy to clients assigned to caseload.
  • Responsible for an active caseload of 50-60 clients.
  • Available for home, hospital and clinic visits.
  • Maintain and establish relationships with providers of services for older adults.
  • Have outstationed hours one day a week at the John C. Anderson Apartments.
  • Participate and facilitate agency sponsored programs at the John C Anderson Apartments
  • Re-assess client needs on a regular basis.
  • Arrange for necessary services.
  • Provide follow-up and monitoring of services client is receiving.
  • Collaborate with medical and social services staff to coordinate services and monitor progress.
  • Collaborate with Action Wellness volunteer department as needed.
  • Participate in weekly supervisory sessions.
  • Participate in staff meetings as scheduled.
  • Serve as a resource for the community around HIV related issues, especially as they relate to older adults.
  • Engage in case finding and outreach efforts as necessary.
  • Collaborate and network with offsite providers are required.
  • Attend a minimum of 2 medical appointments per year for each client.
  • Provide intake coverage as assigned.
  • Other duties as required.

Performance Standards:

  • Provide a minimum of 450 units of service a month for full caseload. (1 units of service = 15 minutes of time spent with a client or on behalf of a client.)
  • Make initial contact with client within three working days of case assignment.
  • Complete initial assessment within 30 days of case assignment.
  • Complete Service Care Plan within 30 days of case assignments.
  • Contact each client at least once a month.
  • Document each service provided to clients within 2 business days of service in CaseWorthy.
  • Maintain client confidentiality.
  • Meet with supervisor for weekly individual supervision once a week.
  • Maintain effective working relationships with other providers.
  • Adhere to core values as outlined in Employee Handbook.
  • Other duties as assigned.
