About the Role:

We are currently seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for providing comprehensive physical therapy services to patients in their homes. You will work closely with a multidisciplinary team to assess patients, develop treatment plans, and help patients achieve their maximum level of function and independence.


Key Responsibilities:

  • Conduct initial assessments and evaluations of patients' conditions and needs
  • Develop individualized treatment plans based on patient assessments and goals
  • Provide skilled physical therapy interventions and treatments to patients in accordance with physician orders and established protocols
  • Educate patients and their families on exercises, activities, and techniques to improve function and mobility
  • Monitor and document patient progress and adjust treatment plans as necessary
  • Collaborate with other healthcare professionals, including nurses, occupational therapists, and social workers, to ensure coordinated and comprehensive care
  • Supervise Physical Therapy Assistants as needed
  • Adhere to all regulatory requirements and company policies and procedures


Qualifications:

  • Graduate from an accredited physical therapy program
  • Current California license as a Physical Therapist
  • Minimum of 2 years of recent experience as a Physical Therapist
  • Current CPR certification
  • Valid California drivers license and automobile insurance
  • Experience in home care, outpatient, or clinic settings preferred
  • Strong communication and interpersonal skills
  • Ability to work effectively both independently and as part of a team
