The Case Manager (CM) provides services to clients with substance use disorders who live in GAAMHA Supportive Housing facilities. These services include a full range of case management activities with a goal of developing self-sufficiency and sustained recovery for the individuals served. The CM is also responsible to ensure that the facilities are properly maintained to reflect the safety, hygiene and appearance standards that GAAMHA requires for all programs.

Responsibilities and Duties:
  • The CM will complete a thorough assessment at intake with client to identify clients’ strengths and service needs.
  • The CM is responsible for completing the Biopsychosocial Assessment and an Individual Service Plan in collaboration with the client within the first week of admission.
  • The CM will administer urinalysis/breathalyzer tests to ensure compliance with program rules.
  • The CM will assist the client in utilizing support systems, appropriate resources, and available community resources.
  • The CM ensures client has transportation to other needed services, such as interviews, medical appointments, court appearances, emergencies, and other community appointments.
  • The CM will be responsible for all related recordkeeping and reports.
  • The CM facilitates weekly house meetings in collaboration with the house manager.
  • The CM will attend all mandatory case conference meetings and training courses.
  • The CM will network and represent GAAMHA Supportive Housing in the community and will behave in a way that reflects the agency's values and professional standards.
  • The CM will adhere to applicable laws, regulations, and agency policies governing the disclosure of client identifying data and confidentiality.
  • Other tasks may be deemed necessary.

Qualifications:
The candidate will have a Bachelor’s Level Degree or demonstrated equivalent experience. Candidates with CADC or LADC and experience in residential treatment settings preferred. The candidate must have a working knowledge of 12 Step programs and other mutual aid support programs. Must have excellent oral, written, and social skills. The candidate will have computer skills and be proficient in MS office. Be able to demonstrate cultural competence and carry out all duties in compliance with HIPAA and 42CFR Part 2. The candidate must hold a valid driver’s license.

Supervision Given: The Case Manager provides supervision, direction, and assistance to the house managers at each sober house under their direction.

Supervision Received: The Case Manager receives administrative supervision from the Assistant Director of Supportive Housing and clinical supervision from the Clinical Supervisor.

Schedule: Monday-Friday 8am-4pm, some nights or weekends, as needed.
